摘要
目的对修订版社区老年人跌倒危险评估工具(Modified Falls Risk for Older People in the Community Screening Tool,MFROP-Com)进行信效度检验。方法使用MFROP-Com对211名社区老年人进行跌倒危险评估,根据评估结果检验该工具的信度和效度,同时利用受试者工作特征(receiver operating charalteristic,ROC)曲线评价其预测准确性。结果 MFROP-Com内部一致性Cronbach’sα系数为0.804;评定者信度良好,护理硕士与社区医生对老年人跌倒危险评估得分差异无统计学意义;各条目得分与总分Pearson相关系数在0.199～0.700之间,呈显著相关;有无跌倒史两组老年人MFROP-Com评分比较,差异有统计学意义(P<0.01);MFROP-Com与修订版跌倒效能量表Spearman相关系数为r=-0.824,呈显著负相关;ROC曲线下面积(Area Under the Curve,AUC)为0.862,当选取临界值为11.50时,工具预测效度的综合评价指标Youden指数最高为0.592,此时灵敏度为0.800,特异度为0.792,阳性预测值为80.7%,阴性预测值为78.4%。结论 MFROP-Com是一种简单、方便、安全的评估工具,其信度和效度较好,适合医护人员用于老年人的跌倒危险评估,并能初步筛选出高危跌倒人群。
Objective To estimate the reliability and validity of Modified Falls Risk for Older People in the Community Screening Tool（MFROP-Com） in fall risk assessment of older people in community.Methods The MFROP-Com was applied among 211 older people for fall risk assessment,the result of which in turn was applied to assess the reliability and validity of it.Meanwhile,prediction accuracy of MFROP-Com was evaluated by ROC.Results Cronbach＇s α coefficient was 0.804,and Pearson＇s correlation coefficient ranged between 0.199 and 0.700,which suggested there were significantly positive relationships among the scores of each item and the total score of MFROP-Com.Spearman correlation coefficient was-0.824,which indicated there was significantly negative relationship between MFROP-Com and Modified Fall Efficacy Scale.Besides,AUC reached 0.862 and maximum Youden index was up to 0.592,sensitivity of 0.800,specificity of 0.792,positive predictive value of 80.7% and negative predictive value of 78.4% once critical value reached 11.50.Conclusion MFROP-Com is proved to be a simple,convenient and safe assessment method with good reliability and validity,which could be applied for the fall risk assessment of older patients.
