摘要
目的探讨胰岛素联合重组人表皮生长因子创面湿敷治疗Ⅲ期及以上压疮的临床效果。方法将40例Ⅲ期及以上压疮病人(98处创面)随机分为两组。实验组在初步清创基础上使用胰岛素联合重组人表皮生长因子湿敷,再予以银离子敷料或水胶体敷料包扎;对照组采用0.5%呋喃西林或1%碘伏纱块湿敷,凡士林油纱敷料包扎。比较两组病人的治疗效果。结果实验组明显缩短压疮创面愈合时间;不同时间创面平均愈合率明显优于对照组,差异均具有统计学意义(P<0.01);同时实验组在外敷胰岛素前后血糖值无明显变化,亦无其他不良反应。结论使用胰岛素联合重组人表皮生长因子创面湿敷治疗Ⅲ期及以上压疮能显著促进创面愈合,缩短愈合时间,且无毒副作用。
Objective To determine the clinical effects of wet spreading treatment of insulin colnbined with recombinant human epidermal growth factor on stagelll or above pressure sore wounds. Methods A total of 40 cases, which had 98 sites with phase Ⅲ or above pressure sore, were randomly divided into two groups. The experimental group received wet spreading treatment with insulin and recombinant human epidermal growth factor on the basis of primary debridement and then binding up wounds with silver dressings or hydrocolloid dressing. The control group received wet spreading treatment with 0.5% Furacilinum or 1% Iodophor gauze blocks, and then binding up wounds with Vaseline gauze blocks. And then to compare the clinical effects. Results The average time of wound healing of the experimental group was significantly lower than the control group, and the healing rale of wounds in different periods was also significantly higher than the control group ,the difference were statisically significant (P 〈0.01 ). At the same time, the experimental group had no obvious change in the blood sugar level before and after external application of insulin, and no other adverse reactions occured. Conclusion The wet spreading treatment of insulin combined with recombinant human epidermal growth factor can significantly enhance the healing of pressure sores above stage Ⅲ, reduce the healing time, and has nontoxic side effects.
