摘要
目的探讨颈深部脓肿营养支持治疗的有效方法。方法对10例颈深部脓肿患者临床资料及营养支持治疗有关资料进行回顾性分析。结果营养治疗前电解质紊乱,血钠(134.0±2.7)mmol/L,血钾(3.5±0.3)mmol/L,血氯(96.2±4.9)mmol/L、低蛋白血症,白蛋白为(34.9±3.4)g/L、白细胞计数增高为(15.8±4.3)×109/L、总淋巴细胞计数降低为(1.2±0.4)×109/L、血红蛋白正常为(128.8±3.9)g/L;营养治疗7d后,电解质紊乱纠正,与营养治疗前比较均有显著性变化,血钠(137.5±2.6)mmol/L(P<0.01),血钾(4.0±0.2)mmol/L(P<0.05),血氯(103.0±2.9)mmol/L(P<0.01),血浆白蛋白水平提高为(37.1±3.2)g/L(P<0.05),白细胞计数下降为(10.3±2.2)×109/L(P<0.01),总淋巴细胞计数升高为(1.8±4.2)×109/L,血红蛋白水平升高为(129.3±3.3)g/L,但统计学无显著性变化;营养治疗14d后,电解质水平维持正常,白细胞计数恢复正常为(7.9±0.7)×109/L,与营养治疗前比较有显著性变化(P<0.01),总淋巴细胞计数继续升高为(2.1±0.2)×109/L(P<0.05),白蛋白为(38.6±2.1)g/L,血红蛋白为(132.5±2.3)g/L,呈上升趋势,均较治疗前呈显著性变化(P<0.01)。结论对颈深部脓肿患者,在去除感染灶和有效合理使用抗菌药物治疗同时,应及时行营养支持治疗,合理选择和调整营养支持方式、营养制剂剂型、营养底物需要量的供给,在胃肠道功能允许的前提下,优选肠内营养治疗。
OBJECTIVE To explore the effective methods of nutritional support therapy in neck abscess.METHODS The data of ten neck abscess patients,related to the clinical therapy and nutritional support therapy were retrospectively reviewed and analyzed.RESULTS Before nutritional support therapy,electrolyte was disordered,Na＋ was（134.0±2.7） mmol/L,K＋ was（3.5±0.3）mmol/L,Cl-was 96.2±4.9mmol/L,hypoproteinemia,albumin was（34.9±3.4）g/L,White Blood Cell number rised was（15.8±4.3）×109/L,lympholeukocyte number reduced to（1.2±0.4）×109/L,hemoglobin was（128.8±3.9）g/L,which was normal;After 7 days of nutritional support therapy,electrolyte disordered was corrected,compared with Before rutritional support therapy,which had remarkable transformation,Na＋ was（137.5±2.6）mmol/L（P〈0.01）,K＋ was（4.0±0.2）mmol/L（P〈0.05）,Cl-was（103.0±2.9）mmol/L（P〈0.01）,albumin rose to（37.1±3.2）g/L（P〈0.05）,White Blood Cell number reduced to（10.3±2.2）×109/L（P〈0.01）,the total lympholeukocyte number rose to（1.8±4.2）×109/L,hemoglobin rose to（129.3±3.3）g/L,but it was not remarkable transformation;After 14 days of nutritional support therapy,electrolyte was nomal,White Blood Cell number was also nomal,which was（7.9±0.7）×109/L,compared with Before nutritional support therapy,its have remarkable transformation（P〈0.01）,lympholeukocyte number rose continually,which was（2.1±0.2）×109/L（P〈0.05）,albumin was（38.6±2.1）g/L,hemoglobin was（132.5±2.3）g/L,which presented rising tendency,compared with the nutritional support therapy,all have remarkable transformation（P〈0.01）.After active clinical therapy and nutritional support therapy,infections of all patients were controlled rapidly,nutritional conditions were resumed rapidly,all patients were clinical cured,there no accidents and complications were occurred.CONCLUSION When infectious messes are dispelled,antibiotics are reasonably and effectively used.Neck abscess patients should be nutr
