摘要
目的探讨前列地尔脂微球注射液治疗急性脑梗死的疗效及其对血浆肿瘤坏死因子-α(TNF-α)的调节作用,并评价其临床意义。方法选择急性脑梗死患者64例,随机分为常规治疗组和前列地尔组,每组32例,常规治疗组给予拜阿司匹林75 mg,普伐他汀10 g,神经节苷脂20～40 mg,奥拉西坦4～6 g,丹参注射液20 ml溶于0.9%氯化钠注射液250 ml或5%葡萄糖注射液250 ml中静脉滴注,1次/d,疗程为14 d。治疗组在常规治疗基础上应用前列地尔脂微球注射液10μg,加入0.9%氯化钠溶液100 ml中静脉滴注,2次/d,疗程为14 d。采用双抗体夹心酶联免疫(ELISA)法于治疗前后测定两组患者的血浆TNF-α的水平。结果治疗后两组患者的神经功能缺损评分、日常生活能力评分间差异均有统计学意义(P<0.01),且前列地尔组的TNF-α水平显著低于常规治疗组(P<0.05)。两组患者的疗效间差异有统计学意义(P<0.01)。结论加用前列地尔脂微球注射液治疗急性脑梗死的疗效明显优于常规治疗的疗效,前列地尔脂微球注射液能降低血浆TNF-α水平,有效改善患者神经功能症状,是治疗急性脑梗死的有效药物。
Objective To observe the regulating effects of intravenous dripped Lipo-PGE1 on plasma tumor necrosis factor-α（TNF-α） levels in patients with acute cerebral infarctions and to evaluate the clinical significance.Methods A total of 64 patients with acute massive hemispheric infarctions were divided into two groups randomly：the treatment group and the control group（32 cases each）.Both groups received intravenous injection of Bayaspirin75 mg,Pravastatin10 g,Oxiracetam4~6 g,and Danshen injection 20 ml diluted by 250 ml normal saline（NS） or glucose（5%） once per day for 14 days.The treatment group received additional 10 μg Lipo-PGEl diluted by 100 ml NS intravenously twice a day for 14 days.Plasma TNF-α concentrations were determined by ELISA method in both groups before and after treatment.Results NDS and ADL scores showed significant differences between the two groups after treatment（P0.01）.Plasma TNF-α concentrations in the treatment group were remarkably lower than in the control group（P0.05）.Curative effects between the two groups showed statistical differences（P0.01）.Conclusion Additional intravenously injected Lipo-PGE1 decreases the plasma TNF-α concentration and improves the neural functions of in acute cerebral infarction patients,which makes it an effective drug superior to conventional drugs.
