摘要
目的:观察肥胖型多囊卵巢综合征(PCOS)患者血清炎症因子的变化,探讨二甲双胍干预后的临床效果。方法:入选63例PCOS患者根据体重指数(BMI)分为肥胖型PCOS(BMI≥25 kg/m2)30例和非肥胖型PCOS(BMI<25 kg/m2)33例;另入选62例同期年龄相匹配的非PCOS育龄妇女为对照组,根据BMI分为肥胖对照组(BMI≥25 kg/m2)30例和正常对照组(BMI<25 kg/m2)32例。检测各组血清卵泡刺激素(FSH)、黄体生成素(LH)、泌乳素(PRL)、雌二醇(E2)、睾酮(T)和炎症因子高敏C反应蛋白(hsCRP)、肿瘤坏死因子-α(TNF-α)水平的变化。肥胖型PCOS患者给予二甲双胍干预12周,观察上述血清指标治疗后的变化。结果:与对照组比较,PCOS患者T、hsCRP和TNF-α水平升高,肥胖型PCOS高于非肥胖型PCOS,非肥胖型PCOS又高于肥胖对照组,而肥胖对照组高于正常对照组(均P<0.05)。经二甲双胍治疗12周后,肥胖型PCOS患者血清T、hsCRP和TNF-α水平明显下降(均P<0.05)。结论:肥胖型PCOS患者血清炎症因子水平升高,二甲双胍可降低肥胖型PCOS患者血清炎症因子水平。
Objective: To observe the changes of serum inflammatory factors in obese patients with polycystic ovary syndrome (P- COS) , explore the clinical efficacy after metformin intervention. Methods: 63 patients with PCOS were selected, then they were divided into obese PCOS group (BMI ≥25 kg/m2, 30 patients) and nonobese PCOS group (BMI 〈 25 kg/m2, 33 patients) . 62 age matched women of childbearing age without PCOS at the same time were selected as control group, then they were divided into obese control group ( BMI ≥ 25 kg/m2 ,30 patients) and normal control group (BMI 〈25 kg/m2, 32 patients) . The changes of serum levels of follicle stimulating hormone ( FSH), prolactin, estradiol, testosterone and high sensitive C - reactive protein, tumor necrosis factor - a ( TNF - a) in the four groups were detected. The patients in obese PCOS group were treated with metformin for 12 weeks, the changes of above - mentioned indexes after treatment were observed. Results: Compared with control group, the levels of testosterone, high sensitive C - reactive protein and TNF - a in patients with PCOS increased, the levels of above - mentioned indexes in obese PCOS group were higher than those in non - obese P- COS group, the levels of above - mentioned indexes in non - obese PCOS group were higher than those in obese control group, and the levels of above - mentioned indexes in obese control group were higher than those in normal control group ( all P 〈 0. 05) . After treated with metformin for 12 weeks, the serum levels of testosterone, high sensitive C - reactive protein and TNF - a in obese PCOS group decreased significantly ( all P 〈 0. 05 ) . Conclusion: The serum levels of inflammatory factors increase in the obese patients with PCOS, metformin can reduce the serum levels of inflammatory factors in the obese patients with PCOS.
