影响肝移植术后早期急性肾功能不全相关危险因素分析 被引量:3

AN ANALYSIS OF RISK FACTORS LEADING TO ACUTE RENAL FAILURE IN EARLY LIVER TRANSPLANTATION

摘要 目的探讨影响肝移植术后早期急性肾功能不全的相关危险因素。方法回顾性分析我院肝移植中心2008年3月~2009年10月间资料完整的99例肝移植患者围手术期病例资料,利用回归分析方法筛选肝移植术后急性肾功能不全(ARF)的相关因素。结果手术早期ARF患者29例(29.3%),在回归分析中患者肝移植术前血清肌酐水平是肝移植术后急性肾功能不全独立危险因素(P=0.004);FK506日用量是术后发生ARF独立危险因素(P=0.031);结论术前血清肌酐、术后FK506日用量是影响肝移植早期急性肾功能不全的独立危险因素。 Objective To explore risk factors leading to acute renal failure in early liver transplantation.Methods Clinical data of 99 cases undergoing liver transplantation between 2008-2009 in our centre were retrospectively analyzed.Logistic regression analysis were used to determine relative risk factors leading to ARF in liver transplantation.Significant factors were then entered into logistic regression to identify factors associated with ARF in liver transplantation.Result During a fellow-up of 30 days,acute renal failure developed in 29 patients(29.3%).Logistic regression analysis indicated that an increased risk of acute renal failure was associate with pretransplatation Cr level(P=0.004),postoperative dose of Tac(P=0.031).Conclusion There are a lot of factors are contributing to ARF in early liver transplantation.Pretransplatation Cr、postoperative dose of Tac is connective with ARF.
