摘要
目的比较痔上黏膜环切术(PPH)和经肛吻合器直肠切除术(STARR)两种经肛门术式治疗直肠前突的效果。方法回顾性分析2007-01～2009-05期间因直肠前突排便困难接受手术的住院患者的临床资料,根据不同手术途径分为PPH组(n=28)和STARR组(n=20)。对2组患者手术后症状改善情况、手术时间、住院时间、费用等进行比较。结果两组患者随访时间大于24个月,失访5例(PPH组3例,STARR组2例,失访率10.4%)。PPH组和STARR组在手术后3月症状改善情况、便秘评分、住院时间没有显著差异(P>0.05),术后24月症状改善情况、便秘评分STARR组强于PPH组(P<0.05)。术后24月PPH组总有效率(88.0%)低于STARR组(94.4%),但是手术费用和手术时间低于STARR组(P<0.05)。结论与PPH术相比,STARR术治疗直肠前突虽然在手术住院费用和手术时间上有所增加,但远期疗效较好。
Objective To analyze the effectiveness of two kinds of surgery in treatment of rectocele.Methods Forty-eight hospitalized patients with rectocele who received surgery due to difficult defecation were divided into two groups according to the different surgery types：procedure for prolapse and hemorrhoids（PPH）group（n=28） and stapled transanal rectal resection（STARR） group（n=20）.The clinical data,including symptom improvement,operation time,hospitalization time and therapy fee were compared between two groups.Results All patients were followed up for more than 24 months,of whom,5 patients（10.4%） were lost to follow-up,3 in PPH group and 2 in STARR group.There was no significant difference in symptom improvement,hospitalization time and constipation score at month 3 after surgery between 2 groups（P0.05）.But all those indices were statistically different at the end of month 24（P0.05）,and the total effective rate in PPH group was lower than that in STARR group（88.0% vs 94.4%）.The operation expense and time of PPH were also lower than that of STARR.Conclusion Compared with PPH,STARR has a better long-term efficacy although its fee is higher and surgery time is longer.
