摘要
目的分析诈病临床特点,帮助基层医生迅速识别诈病,提高诈病的诊断率。方法对急诊科2005年1月1日~2010年12月31日以昏迷为主诉就诊患者进行统计分析。结果以昏迷为主诉就诊患者共有301例,其中确诊为诈病患者10例,占总数3.3%。诈病患者血压、脉搏、呼吸、体温等生命体征不正常阳性率较昏迷患者低。诈病患者与昏迷患者就诊时均不能被唤醒。诈病患者首次睫反射检查全部阳性,昏迷患者睫反射检查全部阴性。诈病患者对轻度疼痛刺激反应及重度疼痛刺激反应阳性率均较昏迷患者低,诈病患者眼球转动或躲避阳性率较昏迷患者高。诈病患者瞳孔大小及对光反应检查不满意。结论睫毛反射及疼痛刺激检查简单易行,结果也比较客观,有助于基层医生迅速识别诈病,提高诈病的诊断率,值得在临床上推广应用。
Objective Analyzed Cheat disease clinical characteristics,in order to help basic-level doctors quickly identifying cheat disease,Improving cheat disease diagnosis rate.Methods Statistical analysis hospital patients with complained of coma from 2005-01-01 to 2010-12-31 in our hospital emergency department.Results There are 301 hospital patients with complained of coma from 2005-01-01 to 2010-12-31 in our hospital emergency department including 10 cases diagnosed cheat disease,accounting 3.3 percent for total hospital patients with complained of coma.The positive rate in Cheat patients with not normal Vital signs such as blood pressure and pulse and respiration is smaller than in coma.The Cheat patients and coma cannot be awakened when they see a doctor.All of the first lashes reflection check is positive in cheat patients,but all of the continuous tracing lashes reflection check is negative in cheat patients.All of the lashes reflection check are negative in coma patients.The positive rates of mild pain stimulation reactions and severe pain stimulation reactions in Cheat patients are all smaller than in coma.All eyeball turning or avoiding in Cheat patients is positive,All eyeball turning or avoiding in coma is negative.The check of pupillary size and light reaction are not satisfied in cheat patients.Conclusion The check of Lashes reflection and pain stimulation reactions is simple,and the results are also objectively.It can conduce to the grassroots doctors quickly identifying cheat disease,improving the diagnosis of disease.It is worth being promoted in clinical application.
