摘要
目的探讨应用便携式微量泵经脾静脉置管输注肝素,防治断流术后门静脉血栓形成的安全性和有效性。方法 2008年3月—2011年1月门静脉高压症行脾切除加贲门周围血管离断术患者52例,随机分为2组,治疗组30例,对照组22例。治疗组行脾静脉留置导管,术后经便携式微量泵持续输注肝素。对照组餐后口服肠溶阿司匹林(50mg,3次/d),同时静脉滴注复方丹参(16mL/d)和低分子右旋糖酐(500mL/d)。比较2组患者的血小板计数、腹腔引流液量、导管留置时间和凝血酶原时间、门静脉血栓形成等情况。结果 2组术后血小板计数、腹腔引流液量、导管留置时间、凝血酶原时间等差异无统计学意义(P>0.05)。治疗组血栓形成率约为3.3%,对照组高达36.4%,2组差异有统计学意义(P<0.05)。结论应用便携式微量泵经脾静脉置管输注肝素,是预防、诊断和治疗门静脉高压症断流术后门静脉血栓形成的简便而有效的方法。
Objective To investigate the safety and efficiency of the method that persistently given heparin by micro - infusion pump of into spleen vein for prevention and treatment of portal vein thrombosis after splenectomy and devasculation. Methods From March 2008 to January 2011,52 patients after splenectomy and devasculation were divided into two groups （22 cases in the control group, and 30 cases in the theropy group）. The therapy group was persistently given heparin by portable micro - infusion pump into spleen vein. The control group was given Aspirin 50mg 3/d after meal to take orally and intravenous drop of Compound Danshen 16mL/d and Dexttrao 500mL/d. The platelet count, volume of drainage from peritoneal cavity, detaining time of duct and prothrombin time, portal vein thrombosis were examined. Results There was no peritoneal cacity, detaining time of duct significant difference in platelet count, volune of drainage from and prothrombin time. Only 1 case （3.3%） developed portal vein thrombosis in therapy group and 8 cases （ 36.4% ） developed portal group. Conclusion It is a handy and effective method to prevent and treat splenectomy and devasculation that persistently given Heparin by portable spleen vein. vein thrombosis in control portal vein thrombosis after micro- infusion pump into
