摘要
目的评价拔牙术前口服药物对高血压患者血压控制的效果。方法计算机检索MEDLINE,Embase,Cochrane图书馆,SinoMed,VIP,CNKI和WANFANG数据库中关于拔牙术前口服药物对高血压患者血压控制效果的随机对照试验,同时手检纳入研究的参考文献。有2名评价者独立进行文献质量评价和资料提取后,采用RevMan5.1软件进行meta分析。结果共纳入4个研究,包括6个试验958例患者。Meta分析结果显示:与对照组相比,服药后能有效降低舒张压(SMD=-1.23,95%CI:-1.65～-0.80;P<0.01)和心率(SMD=-0.40,95%CI:-0.72～-0.07;P=0.02),差异具有统计学意义;收缩压获得降低但差异无统计学意义(SMD=-0.22,95%CI:-0.63～0.19;P=0.30)。结论拔牙术前口服药物对高血压患者血压控制的有效性已得到初步肯定,但鉴于研究的局限性,建议严格按照CONSORTS标准设计高质量、大样本的随机盲法对照试验对其有效性进一步验证,并对其安全性进行评价。
Objective To review the effect of oral medicinal in blood pressure control in hypertensive patients before exelcymosis.Methods The randomized controlled trials were retrieved with computers about the effect of oral medicinal in blood pressure control in hypertensive patients before exelcymosis from MEDLINE,Cochrane Library,SinoMed,VIP,CNKI and WANFANG,and at the same time the references included the study were manually searched.The documents and materials were analysed by using software RevMan 5.1 and Meta-analysis after the quality evaluation and material extraction by 2 independent reviewers.Results There were totally 4 studies brought into including 958 patients in 6 trails.The results of Meta-analysis showed that medicinal intake decreased effectively diastolic blood pressure（SMD=-1.23,95% CI：-1.65～-0.80;P0.01） and heart rate（SMD=-0.40,95% CI：-0.72～-0.07;P=0.02）,and the difference was significant statistically.The systolic blood pressure was decreased too but the difference was not significant（SMD=-0.22,95% CI：-0.63～0.19;P=0.30）.Conclusion The effectiveness of oral medicinal has been affirmed in blood pressure control in hypertensive patients before exelcymosis,but the study has its limitations.A randomized,blinding and controlled trail,designed according to CONSORT standard with higher quality and large sample,is needed to test the effectiveness further and review the safety.
