摘要
目的:比较采用椎板切除术治疗退行性颈椎管狭窄前后X线平片变化情况。方法:采用椎板切除术治疗退行性颈椎管狭窄,采用颈椎侧位X片测定治疗前后椎体矢状径、椎管矢状径、椎管矢状径/椎体矢状径的比值。结果:治疗前男女患者椎管矢状径/椎体矢状径的比值分别为0.74∶1.00和0.73∶1.00,二者比较,差异无统计学意义(P>0.05);治疗后男女患者椎管矢状径/椎体矢状径的比值分别为0.85∶1.00和0.87∶1.00,二者比较,差异无统计学意义(P>0.05);治疗后椎管矢状径较治疗前变宽,与治疗前比较,差异有统计学意义(P<0.05);治疗后椎体矢状径与治疗前比较,差异无统计学意义(P>0.05)。治疗后椎管矢状径/椎体矢状径的比值较治疗前增大,前后比较,差异有统计学意义(P<0.05)。男性患者和女性患者C3、C4、C5、C6节段椎管矢状径治疗后长于治疗前,差异有统计学意义(P<0.05);男性患者和女性患者C3、C4、C5、C6节段治疗前椎管矢状径长度比较,差异有统计学意义(P<0.05),男性患者椎管矢状径长于女性。结论:退行性颈椎狭窄行椎板切除术前后的X线平片变化能够评价手术的疗效,并且简单方便,值得临床应用。
Objective:To compare the X-ray changes for degenerative cervical spinal stenosis before and after laminectomy.Methods:Degenerative cervical spinal stenosis were treated with laminectomy.Sagittal diameter of spinal canal,sagittal di-ameter of edge or degenerative cervical body and ratio of them were measured before and after laminectomy.Results:The ratio for sagittal diameter of spinal canal and sagittal diameter of edge or degenerative cervical body in male an female patients before(0.74∶1.00,0.73∶1.00) and after(0.85 ∶1.00,0.87∶1.00) laminectomy was not significantly different(P0.05).The sagittal diameter of spinal canal was wider after laminectomy than before(P0.05);the sagittal diameter of edge or de generative cervical body had no significant changes after laminectomy(P0.05).The ratio for sagittal diameter of spinal canal and sagittal diameter of edge or degenerative cervical body significantly increased after laminectomy(P0.05).The sagittal diameter of spinal canal of C 3,C 4,C 5,C 6 in male and female patients were longer than before(P0.05),and sagittal diameter of spinal canal of male patients were longer than female patients before caminectomy(P0.05).Conclusion:The X-ray changes for degenerative cervical spinal stenosis before and after laminectomy can evaluate the outcome of surgical treatment,it is a simple and reliable method and is worthy of clinical popularizing and application.
