摘要
目的探讨"双管法"一体式倒Y型支架置入术治疗气管复合狭窄的可行性和疗效。资料与方法在透视下对2例气道复合狭窄的患者先经鼻腔置入通气导管,再经口-咽-气管释放气道支架。结果 2例患者支架置入均一次成功,在气管支架置入过程中,未出现出血、窒息。术后患者呼吸困难明显改善,气促评级由Ⅳ级改善为0级或Ⅰ级,脉氧饱和度(SPO2)由高流量吸氧时的70%、75%分别提高至自然呼吸时的90%、95%。结论在一体式倒Y型气管支架置入过程中,预先留置通气导管以维持呼吸和供氧,缓解了患者由于插入支架输送器引起和加剧的呼吸困难,能够更好的配合,使得气管支架放置更加安全,其技术可行,操作简单,值得临床推广应用。
Objective To investigate the feasibility and therapeutic effect of double-barrelled inverted Y shaped stent in treatment of airway complex stenosis.Materials and Methods Under the fluoroscopic guidance,2 double-barrelled stents were implanted in 2 cases of airway complex stenosis through nasal cavity.Results The stents were placed successfully without dyspnea and bleeding Occurred.The hypoxia symptom was relieved immediately after stents placement.The dyspnea score improved significantly from Ⅳ to 0 orⅠlevel,and saturation of pulse blood oxygen（SPO2） elevated from 70%,75% to 90%,95%.Conclusion The placement of inverted Y shaped double-barrelled stent is safe and feasible in treatment of airway complex stenosis.
