摘要
目的探讨320排容积CT在全上腹部器官扫描中的优势及临床应用价值。资料与方法 96例患者注射对比剂后,利用Toshiba 320排CT进行上腹部容积扫描。将数据导入Toshiba体部灌注软件对全上腹部器官进行灌注分析,同时选择最佳时相重组平扫、增强各期图像,重组动脉CT血管造影(CTA)、静脉及门静脉CT静脉成像(CTV)。并分组对一站式检查进行辐射剂量分析。结果 89例获取良好的平扫及多期增强容积图像、动态DSA图像、门静脉CTV以及全上腹部低剂量灌注图像。连续动态成像提示动脉期成像最佳时间点分布于19~22 s之间,门静脉最佳成像时间点分布于33~36 s之间。其中发现合并或单发血管病变累计57处;发现合并或单发上腹部器官异常灌注72处;剂量对比发现辐射剂量轻度增加。结论 320排容积CT上腹部一站式检查可以动态观察所有脏器平扫至增强各期血流灌注情况,同时对病灶本身、整个脏器及邻近脏器灌注情况进行定性、定量分析,能排除个体差异影响,准确选择最佳时相显示动脉、静脉系统血管形态及动态血流信息,真正实现了上腹部一站式容积扫描,对于上腹部病变具有极高的临床诊断价值。
Objective To evaluate the clinical advantage and value of upper abdominal one-stop examination with 320 detector row CT.Materials and Methods After having injected the contrast medium,dynamic volume scanning was performed for all 96 patients with Toshiba 320 detector row CT for upper abdominal organs one stop examination.The data were analyzed with Toshiba body perfusion software.The plain and contrast axial thick layer images,CTA,vein and Portal vein CTV were recombined by selecting the data of optimal time phase.Results 89 cases obtained good quality images of plain and contrast,CTA,vein and portal vein CTV,upper abdominal organs perfusion.The dynamic enhanced imaging showed that the optimal time point of arterial phase and portal vein phase distributed in 19.0-22.0 s,33.0-36.0 s respectively.Using these time points,we found vascular diseases in 57 cases,abnormal perfusion in 72 cases.Conclusion Upper abdominal one stop examination using 320 detector row CT can show the anatomy and dynamic blood flow information of organs,vessels exactly by selecting the optimal time phase,which has important clinical value in diagnosis of upper abdominal diseases.
