摘要
目的探讨感染性骨髓象患者外周血中性粒细胞(NE)体积(V)、电导率(C)、激光散射(S)参数(VCS参数)的临床意义。方法利用Beckman-Coulter LH750全自动血液分析仪VCS技术测定29例感染性骨髓象患者以及37例健康者NE平均体积(NE-V)、平均电导率(NE-C)、平均激光散射(NE-S)及相应标准差(NE-VSD、NE-CSD、NE-SSD)。结果感染性骨髓象患者组外周血NE-V、NE-VSD、NE-CSD、NE-SSD高于健康对照组,NE-C、NE-S低于健康对照组(P<0.05)。外周血中性杆状核粒细胞百分含量(Nst%)大于或等于20%的核左移组NE-V高于Nst%小于20%的核左移组(P<0.05),NE-VSD、NE-C、NE-CSD、NE-S、NE-SSD差异均没有统计学意义(P>0.05)。WBC≤10×109/L组NE-V、NE-VSD、NE-CSD、NE-SSD和NE%≤70%组的NEV-SD、NEC-SD、NES-SD以及WBC≤10×109/L合并NE%≤70%组的NEV-SD、NES-SD与健康对照组增高比较均增高,差异均有统计学意义(P<0.05);WBC≤10×109/L组的NE-C、NE-S和NE%≤70%组的NE-C、NE-S以及WBC≤10×109/L合并NE百分含量小于或等于70%组NE-C、NE-S低于健康对照组(P<0.05)。结论外周血NE VCS参数在感染性骨髓象患者中能特异而灵敏地反映粒细胞的形态学变化。
Objective To investigate the change and the clinical significance of white blood cell(WBC) volume,conductivity and laser scatter(VCS) parameters in the patients with infection myelogram.Methods Beckman-Coulter LH750 with VCS parameters were used to detect the volume,conductivity and laser scatter of neutrophils(NE-V,NE-C,NE-S)and the standard error(SD) of these parameters(NE-VSD,NE-CSD,NE-SSD) of 29 patients with infection myelogram and 37 healthy subjects.Results Compared with the healthy control group,the patients group were with higher NE-V,NE-VSD,NE-CSD and NE-SSD(P〈0.05),but lower NE-C and NE-S(P〈0.05).The NE-V in the group of neutrophil left shift,with the percentage of neutrophil stab granulocytes(Nst%) equal with or higher than 20%,increased significantly(P〈0.05),compared with the group of neutrophil left shift with Nst%〈20%,but the NE-VSD,NE-C,NE-CSD,NE-S and NE-SSD were not significantly different(P〉0.05).The NE-V,NE-VSD,NE-CSD and NE-SSD in the group with WBC≤10×109/L,the NEV-SD,NEC-SD and NES-SD in the group with NE%≤70%,and the NE-VSD and NE-SSD in the group combined with WBC≤10×109/L and NE%≤70% were higher than those of healthy controls(P〈0.05),but the NE-C and NE-S in the group with WBC≤10×109/L,the NE-C and NE-S in the group with NE%≤70%,and the NE-C and NE-S in the group combined with WBC≤10×109/L and NE%≤70% were lower(P〈0.05).Conclusion VCS parameters could sensitively and specifically reflect the morphologic changes of neutrophil in patients with infection myelogram
