左室射血分数正常心力衰竭者的临床特点分析 被引量:5

摘要 目的:探讨左室射血分数正常心力衰竭(Heart failure with normal ejection fraction,HFNEF)的临床特点。方法:2009年1月至12月入我院治疗的慢性心力衰竭患者91例,根据2007年欧洲心脏病协会的HFNEF专家共识诊断HFNEF,给予抗心衰治疗,随访1年。结果:91例患者中,射血分数≥50%的HFNEF患者共48例(52.7%)。HFNEF患者较射血分数<50%的射血分数减损的心力衰竭(Heart failure with deteriorated ejection fraction,HFDEF)患者相比较,女性、高血压性心脏病和心功能Ⅱ级较多,合并高血脂和吸烟的也多。心超检查显示,除射血分数EF外,HFNEF组患者LVDd、LVDs、LAD及LVOT均低于HFDEF组,而HFNEF组患者FS高于HFDEF组患者。胸片显示HFNEF组心脏扩大和肺淤血的改变较少。HFNEF患者1年内再入院率和死亡率均低于HFDEF患者。结论:有50%以上的慢性心力衰竭患者保持有正常的左室射血分数,其中高血压、女性患者更为常见,心室扩大程度较轻,再入院率和死亡率也较低。 Objective:To investigate the clinical characteristics of heart failure with normal ejection fraction(HFNEF).Methods:From January 2009 to December 2009,91 patients in our hospital with chronic heart failure were enrolled.According to a 2007 European Society of Cardiology expert consensus,HFNEF were diagnosed.The patients were given anti-heart failure therapy and at least 1-year follow-up.Results:A total of 48 patients had ejection fraction 50% of the HFNEF(52.7%) in 91 patients.Compared with heart failure with deteriorated ejection fraction(HFDEF) patients,HFNEF included more women,more cases with hypertensive heart disease and Ⅱ level heart function,and were combined more with hyperlipidemia and closely related to smoking rate.Echocardiography showed LVDd,LVDs,LAD,and LVOT in HFNEF group were lower than those in HFDEF group,while EF and FS in HFNEF were higher than those of HFDEF group.X x-ray examination showed changes in chest and lung congestion and cardiac dilatation were smaller in HFNEF.The 1-year readmission rate and mortality rate in HFNEF patients were lower than those in HFDEF patients.Conclusion:Nearly 50% of chronic heart failure patients have a normal left ventricular ejection fraction,more common in female patients or patients with hypertension,in which ventricular enlargement is to a lesser extent,and readmission rate and mortality are low.
