摘要
目的探讨自噬基因Beclin 1在膀胱移行细胞癌(BTCC)组织中的表达和其在膀胱癌发生发展中的作用及预后的相关性。方法免疫组化EnVision法检测58例BTCC组织和20例正常膀胱组织中Beclin 1的表达,分析其与BTCC的分级、分期、侵袭及转移等生物学行为的关系,并对BTCC患者进行随访以期了解Beclin 1对患者生存率的影响。结果 Beclin 1在58例BTCC组织中阳性表达率明显低于正常膀胱黏膜组织中的表达(χ2=9.24,P<0.01)。Beclin 1的表达与病理分级、TNM分期、生长方式、淋巴结转移有关(P<0.05)。Beclin 1表达阳性患者的5年生存率明显优于Be-clin 1表达阴性的患者(P<0.01)。Cox回归多因素分析表明Beclin 1表达是与患者5年生存率相关的独立预后因素(P<0.05)。结论 Beclin 1在BTCC组织中表达下调,并可以作为BTCC预测发生发展和判断预后的生物学标志物。
Objective To investigate the role of Beclin 1 in the tumorigenesis,development and prognosis of bladder transitional cell carcinoma.Methods In 58 cases of BTCC and 20 cases of normal tissue bladder,Beclin 1 expression was examined with immunohistochemical and the correlation of Beclin 1 expression with the invasion,metastasis,angiogenesis and recurrence of BTCCs was analyzed.Results The positive rates of Beclin 1 expression were lower than that in normal bladder mucosa tissues(P0.05).The expression level of Beclin 1 was closely correlated with histological types,TNM staging,lymph node metascasis and growth patterns(P0.05).The survival rate for five years in Beclin 1 negative group was better than that in Beclin 1 positive group.Multivariate analysis with the Cox proportional hazards model showed that Beclin 1 was an independent prognostic factor for bladder transitional cell carcinoma.Conclusions The expression of Beclin 1 protein in bladder transitional cell carcinoma were low.Beclin 1 may act as marker predicting tumorigenesis,development and prognosis of papillary thyroid carcinoma.
