摘要
目的观察超声乳化人工晶状体植入联合小梁切除术治疗青光眼合并白内障的临床疗效。方法回顾性分析我院2005年6月至2009年12月青光眼合并白内障患者32例(32眼),均采用超声乳化人工晶状体植入联合小梁切除术治疗,观察患者术后1d、1周、1个月及6个月的眼压、视力情况,另外记录视野、滤过泡及并发症情况。结果患者术后1d、1周、1个月及6个月的平均眼压分别为(19.2±1.6)mmHg(1kPa=7.5mmHg)、(18.4±1.4)mmHg、(15.6±0.8)mmHg、(15.0±0.7)mm-Hg,与术前(23.2±1.1)mmHg比较,差异均有统计学意义(均为P<0.05)。术前平均视力为0.12±0.08,术后1d、1周、1个月及6个月分别为0.26±0.09、0.28±0.09、0.32±0.12、0.45±0.18;术后视力较术前明显提高,差异均有统计学意义(均为P<0.05)。术前、术后平均视野缺损值与平均模式标准差值差异均无统计学意义(均为P>0.05)。术后形成功能型滤过泡22眼,非功能型10眼。术中出现晶状体后囊膜破裂2眼,均未出现玻璃体脱出等其他并发症。结论超声乳化人工晶状体植入联合小梁切除术治疗青光眼合并白内障既可降低眼压、改善视力,又能减少患者的经济负担,且成功率较高,并发症少。
Objective To investigate the therapeutic effect of phacoemulsification and intraocular lens implantation combined with trabeculectomy for glaucoma and cataract.Methods Thirty-two patients（32 eyes） with glaucoma and cataract from June 2005 to December 2009 in our hospital were retrospectively analyzed,all cases received phacoemulsification and intraocular lens implantation combined with trabeculectomy,the intraocular pressure and visual acuity at postoperative 1 day,1 week,1 month and 6 months were observed,and the visual field,filtering bleb and complications were also recorded.Results The mean intraocular pressure at postoperative 1 day,1 week,1 month and 6 months were（19.2±1.6）mmHg（1 kPa=7.5 mmHg）,（18.4±1.4）mmHg,（15.6±0.8）mmHg,（15.0±0.7）mmHg,compared with that before operation（23.2±1.1）mmHg,there were statistical differences（all P0.05）.The visual acuity before operation and postoperative 1 day,1 week,1 month and 6 months were 0.12±0.08,0.26±0.09,0.28±0.09,0.32±0.12 and 0.45±0.18;The visual acuity after operation was higher than that before operation,there were statistical differences（all P0.05）.There was no difference in mean visual field defect value and standard mode difference value before and after operation（all P0.05）.The functional filtering blebs were formed in 22 eyes after operation,and the non-functional blebs in 10 eyes.The posterior capsule rupture appeared in 2 eyes after operation,no other complications occurred.Conclusion Phacoemulsification and intraocular lens implantation combined with trabeculectomy for glaucoma and cataract not only decreases the intraocular pressure and improve the visual acuity,but reduce the economic burden of patient with high successful rate and less complications.
