摘要
目的评价经皮氧分压(TcPO2)监测在临床麻醉中的应用价值。方法对28例在全麻下实施骨科手术的患者进行持续TcPO2监测,分别在吸入氧浓度(FiO2)为21%、40%、70%与100%时进行动脉血气分析测定,并与动脉血氧分压(PaO2)进行对比,比较4组不同FiO2的经皮氧分压指数(TcPO2/PaO2)。结果在不同吸入氧浓度的条件下,TcPO2与PaO2之间均呈显著正相关,FiO2为21%时TcPO2与PaO2相关系数r=0.599(P<0.01),FiO2为40%时r=0.715(P<0.01),FiO2为70%时r=0.718(P<0.01),FiO2为100%时r=0.708(P<0.01)。4组TcPO2/PaO2无显著性差异(P=0.063)。结论 TcPO2能够较准确地反映PaO2的变化趋势,并且TcPO2/PaO2可有效地反映组织灌注,经皮氧分压监测技术在临床麻醉中应用具有可行性。
Objective To evaluate the application value of transcutaneous partial pressure of oxygen(TcPO2) monitoring in clinical anesthesia.Methods Twenty-eight patients who underwent general anesthesia and orthopedics surgery were studied.The inhaled oxygen concentrations(FiO2) were 21%,40%,70% and 100%,and the TcPO2 were measured with TC devices and arterial blood gas analysis were tested simultaneously.Measurements of TcPO2 were compared with arterial blood oxygen pressure(PaO2) in different inhaled oxygen concentrations respectively,and four groups of transcutaneous oxygen indexes(TcPO2/PaO2) were compared.Results Although inhaled oxygen concentrations were different,TcPO2 correlated positively with PaO2.When FiO2 was 21%,the correlation coefficients(r)for TcPO2 and PaO2 was 0.599(P 0.01);while when FiO2 was 40%,70% and 100%,the r was 0.715(P 0.01),0.718(P 0.01),and 0.708 respectively(P 0.01).There was no significant difference in four groups in TcPO2/PaO2(P = 0.063).Conclusions TcPO2 reflects the trend of PaO2 change more accurately and TcPO2/PaO2 can effectively reflect tissue perfusion.Transcutaneous partial pressure of oxygen monitoring in clinical application of anesthesia is feasible.
