摘要
目的探讨动力髋螺钉(DHS)治疗老年股骨粗隆间骨折的方法及疗效。方法对2005年3月至2010年12月许昌市中心医院骨科收治的老年股骨粗隆间骨折患者78例,根据患者的病情,选择动力螺钉(DHS)治疗,并根据随访结果进行评价。结果 78例均得到随访,时间8～24个月,平均16个月,按Sanders等创伤性髋关节评分标准:优42例,良22例,中8例,差6例,优良率为82.1%。本组患者无1例感染,术后2年无1例死亡。全部骨折均骨性愈合。结论 DHS是治疗老年股骨粗隆间骨折的较好选择。掌握手术适应证,选择合适的DHS,娴熟的手术技术及术后指导功能锻炼,可预防发生骨粘连及各种畸形愈合。
Objective To investigate the methods and results of dynamic hip screw（DHS） in treatment of femoral intertrochanteric fractures.Methods The elderly in our hospital patients with intertrochanteric fracture in 78 cases,according to the patient＇s condition,select the power screw（DHS） treatment,and according to follow-up results were evaluated.Results Totally 78 patients were followed up 8 to 24 months,an average of 16 months,according to Sanders and other traumatic hip score criteria：excellent in 42 cases,good in 22 cases,general in 8 cases and poor in 6 cases,good rate of 82.1 %.1 case of this group of patients without infection,after 2 years without 1 death.All fractures healed.Conclusions DHS is a better choice in treatment of intertrochanteric fractures of elderly.To master surgical indications,select the appropriate DHS,skilled guidance of surgical technique and postoperative functional exercise,can prevent the occurrence of adhesions and a variety of abnormalities of bone healing.
