摘要
目的探讨急诊全身性炎性反应综合征(SIRS)患者评分与凝血功能变化关系,为临床提供合理SIRS病情评估,指导治疗。方法采用前瞻性方法收集2009年11月1日至12月30日到广西医科大学东院急诊科急诊符合SIRS诊断标准的患者218例,进行评分分组并测定患者血小板(PLT)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(Fg)和D-二聚体(DD)值,比较不同评分分组间差异性及相关性。结果 SIRS患者的凝血系统指标中,随着SIRS评分升高,PLT和Fg逐渐减少(P<0.01),PT、APTT和DD逐渐延长和增加(P<0.01);SIRS患者评分与PLT和Fg含量呈负相关,与PT、APTT和DD含量呈正相关。结论 SIRS凝血功能紊乱程度与疾病严重程度具有明显相关性,凝血纤溶系统活化指标PLT、PT、APTT、Fg、DD均可作为反映SIRS患者病情危重程度的参考指标,在SIRS评分基础上结合SIRS患者的凝血功能动态检测,从而更好地进行SIRS病情评估,指导治疗。
Objective To explore relationship between systemic inflammation response syndrome patient grading and hemoglutination function change in emergency department,in order to provide the reasonable SIRS condition appraisal and the clinical instruction treatment.Methods First supports the nature of the method to collect conform to the SIRS diagnosis standard 218 patients from November 1 to December 30,2009 in emergency department of east hospital The first Clinical Medical College of Guangxi Medical University,carries on grading to group and to determine the patient blood platelet（PLT）,prothrombin time（PT）,activated partial thromboplastin time（APTT）,fibrinogen（Fg）and D-the dimer（DD）value,compared with different grading grouping difference and relevance.Results In the SIRS patient＇s hemoglutination system target,go up with the SIRS grading,PLT and Fg reduce（P〈0.01）,PT,APTT and DD lengthen and increase（P〈0.01）;SIRS patient grading and PLT and the Fg content assumes negatively correlated,With PT,APTT and the DD content are positively related.Conclusion The SIRS hemoglutination function degree of turbulence and the disease order of severity have the obvious relevance,the hemoglutination filament dissolve target PLT,PT,APTT,Fg and DD to be possible to take reflected that the SIRS patient condition seriously injured degree the norm for reference,unifies the SIRS patient＇s hemoglutination function dynamic examination in the SIRS grading foundation.Thus carries on the SIRS condition is more appraisal and instructing treatment.
