摘要
目的:了解我院2006、2009年围术期抗菌药物预防应用情况,并对其合理性进行比较和评价。方法:采用回顾性方法,对我院2006、2009年部分手术患者围术期预防用抗菌药物情况进行对比分析。结果:2个年度手术患者科室构成比及切口类别构成比相似,抽取样本具有可比性;围术期抗菌药物应用率达100%,均采用了静脉给药方式;2006年以头孢菌素类、青霉素类和喹诺酮类居多,2009年选用第1、2代头孢菌素增多,选用第3代头孢菌素、氟喹诺酮类患者明显减少;术前0.5～2h给药患者比例分别为0%和4.6%,手术时间>3h或失血量>1500mL手术患者中,术中追加用药者比例分别占0%和13.3%;Ⅰ、Ⅱ类切口均以单一用药为主,术后用药时间超过48h者均占90%以上。结论:通过干预,我院围术期抗菌药物选择及联用趋于合理,但用药率过高、用药时机不当和术后用药时间过长问题依然突出。
OBJECTIVE：To probe into the perioperative prophylactic application of antibacterials in our hospital in 2006 and 2009,and to compare and evaluate the rationality of it.METHODS：By retrospective method,the perioperative prophylactic application of antibacterials in hospitalized patients of our hospital in 2006 and 2009 was analyzed comparatively.RESULTS：The constituent ratio of clinical department and incision type of operation cases in 2006 and 2009 were similar and the sample copy was comparable.All patients received antibacterials by intravenous administration during perioperative period;the most frequently used antibacterials were cephalosporins,penicillins and quinolones in 2006,while the application of first and second generation cephalosporins increased in 2009,the application of third generation cephalosporin and quinolones decreased oppositely.The cases of prophylactic application of antibacterials 0.5～2 h before operation accounted for 0% and 4.6%,and the ratios of additional application of antibacterials during operations in which operation lasted for more than 3 h or blood loss exceeded 1 500 mL were 0% and 13.3%,respectively.It was mainly single application of antibacterials in type Ⅰ and type Ⅱ incision,and the cases of application period after operation exceeding 48 h was more than 90%.CONCLUSION：About the perioperative prophylactic application of antibacterials in our hospital,it tends to be reasonable in drug selection and combined application after intervention.There still are problems about high utilization rate of antibacterials,unreasonable medication time and long period of post-operative medication.
