摘要
目的探讨影响石家庄市辖区肺结核病人系统管理的因素。方法对石家庄市疾控中心结核门诊2008-2009年登记和管理病人的相关资料进行整理分析,并用回顾性调查方法对负责病人管理的区、社区服务中心、社区服务站以及市级结核门诊大夫进行问卷调查。结果 322例未按规定满疗程治疗的病人中:128人由于不良反应停药,不良反应的发生率为9.51%,占未满疗程总数的40%;外地户籍者77人,占24%;194人由于各种原因丢失或中断治疗,占总数的60%。结论不良的医患关系(医患之间的信任度低)、人口的流动、结防机构的诊治条件和水平以及对基层的管理激励费低是影响结核病人系统管理的主要因素。
Objective To study the effect factors of tuberculosis patient system management in Shijiazhuang.Method Selection and analysis of registration and management of tuberculosis patients in shijiazhuang CDC TB clinic during 2008 to 2009,using retrospective survey method and carried out questionnaire survey to the doctors of community service centers,community service stations and municipal tuberculosis clinics.Results In 322 did not press formulary full treatment cases,128 persons stopping drug because of adverse reactions,the adverse reaction rate was 9.51%,40% of total un-full treatment patients;77 persons were nonlocal census registers,and accounted for 24%;194 people were absence or cessation treatment because of other reasons,and accounted for 60% of total.Conclusions Bad doctor-patient relationship（little trust between doctor and patient）,population flow,diagnosis conditions and treatment level of tuberculosis dispensary,low management and encouragement fees of grass-roots and all of them were the key factors of effecting the tuberculosis person system management.
