摘要 目的观察短期应用胰岛素泵治疗对初诊2型糖尿病患者血糖、胰岛β细胞功能和胰岛素敏感性的影响。方法用胰岛素泵对72例初诊2型糖尿病患者进行2周的持续皮下胰岛素输注(CSII),观察并记录治疗前后空腹血糖(FPG)及餐后2h血糖(2hPG)、糖化血红蛋白A1c(HbA1c)、空腹胰岛素(FINS)、餐后2h胰岛素(2hINS)、胰岛素分泌指数(HOMA-β)、早期胰岛素分泌指数胰岛素抵抗指数(HOMA-IR)。计算胰岛素及葡萄糖曲线下面积(AUCI,AUCG)。结果 CSII治疗能快速稳定控制血糖,治疗2周后FINS及HOMA-β明显上升,HOMA-IR明显下降(P<0.01)。结论短期CSII治疗对2型糖尿病有快速稳定血糖、改善胰岛β细胞功能和降低胰岛素抵抗的作用。 Objective To observe the effect of short-term continuous subcutaneous insulin infusion ( CSII ) on blood glucose, betacell function and insulin resistance in patients with newly-diagnosed type 2 diabetes mellitus. Methods Seventy-two patients with newly-diagnosed type 2 diabetes mellitus receiving two weeks of CSII therapy were analyzed . The levels of the fasting plasma glucose (FPG),2 hours postpradial glucose (2hPG), and glycosylated hemoglobin Alc( HbAlc ), fast insulin (FINS),2 hours postpradial insulin and insulin secretion indexes ( HOMA-β ) and insulin resistance index ( HOMA-IR ) were determined and compared before and after CSII. Results The level of blood glucose was well stably controlled by CSII. The levels of FINS, HOMA-β were increased significantly two weeks after intensive treatment with CSII. The HOMA-IR were decreased significantly. Conclusion Short-term CSII intensive treatment can stably control the level of blood sugar, improve the function of beta-cell and reduce the resistance to insulin in type 2 diabetic patients.
