摘要
目的：研究电子支气管镜对肺癌的诊断价值,并分析肺癌患者的临床特征、病变部位、病理类型与镜下表现的关系。方法：回顾分析2010年8月至2011年2月150例电子支气管镜检查,经病理组织学证实为肺癌的84例资料。结果：确诊的84例肺癌患者中,男女比例为3.9∶1,男性患者中84.4%有吸烟史。男性患者中多数为鳞癌（44/67,65.9%）,其次是小细胞癌（5/67,7.5%）和腺癌（9/67,13.4%）。女性患者以腺癌为主（5/17,29.4%）。50~69岁为肺癌的高发年龄段（62/84,74.4%）,其中多数为鳞癌,占肺癌患者总数的81.4%。所有病例均为单侧肺部病变,发生率右肺明显高于左肺。病变累及气管及隆突者4例。镜下表现多为直接征象（94.0%）,增生型及浸润型均以鳞癌占多数。结论：电子支气管镜检查为临床早期发现,明确诊断肺癌的有效方法。
Objective： To study lung cancer diagnosed by electronic bronchoscope and analyze the relationship among clinic characteristics,location,pathologic types and the sign under electronic bronchoscope.Methods： Electronic bronchoscope data of 84 lung cancer patients diagnosed by histopathology,which were enrolled from 150 cases of patients detected by electronic bronchoscope from August 2010 to February 2011 were analyzed retrospectively.Results： The features of 84 cases of definite lung cancer were： the male/female ratio was 3.9∶1.Smokers took 84.4%.The most common histological type was squamous cell carcinoma（44/67,65.9%）,followed by small cell carcinoma（5/67,7.5%） and adenocarcinoma（9/67,13.4%）in 67 male patients.The more common histological type was adenocarcinoma （29.4%）in 17 cases of female patients.The more common was 50-69 age section（74.4%）in lung cancer,most of which was squamous cell carcinoma which was 81.4% of all the patients of squamous cell carcinoma.All cases of 84 confined one side of lung.It was considerably higher in the right lung than in the left.The tracheal carina was involved in 4 cases.Among cases of lung cancer＇s features under bronchoscope 94.0% had direct signs.Squamous 84 cell carcinoma was more than the others in the proliferation in the windpipe and the infiltration on the windpipe wall.Conclusion： The bronchoscopic exam is an effective way to make early discovery and definite diagnosis of lung cancer.
