摘要
目的探索运用股骨近段锁定加压钢板(PF-LCP)治疗股骨转子间骨折的手术技术。方法采用PF-LCP治疗股骨转子间骨折36例。先确定股骨颈内螺钉位置,用股骨颈螺钉确定钢板位置完成手术。采用Sander标准评价治疗效果。结果手术时间40~95(52.73±13.68)min,术中失血量80~260(136±66.21)ml。36例均获随访,时间6~36个月。骨折愈合时间12~20(13.67±4.25)周。Sander评分:优19例,良14例,差3例。发生并发症15例,均经保守治疗后痊愈;2例发生髋内翻。结论股骨颈内螺钉位置确定钢板位置的手术技术可以应用于PF-LCP治疗股骨转子部骨折,符合微创技术,但要严格掌握操作程序,且器械有待改进。
Objective To study the surgical technique on treatment of inter-or subtrochanteric fractures with the locking compression plate for the proximal femur(PF-LCP).Methods 36 cases of intertrochanteric fractures were treated with PF-LCP.The operation procedure was as follow:first,to identify the screw position in the neck of femur,then to determine the plate position with screws.Function restoration was evaluated by Sander′s score.Results Average operation time was 40~95(52.73±13.68) min,blood loss was 80~260(136±66.21)ml.36 cases were followed up for 6~36 months,union time were 12~20(13.67±4.25)weeks.According to Sander′s score,19 patients reached excellent results,14 good and 3 fair.15 cases with complications were cured by non-operative methods.2 cases were complicated with coax vara.Conclusions The technical can be used in treating intertrochanteric fracture with PF-LCP,which determines the plate position by screws in the neck of femur.It belongs to MIPPO technology.But the technical procedures and key points as well should be demanded strictly,and the instrument should be improved.
