左炔诺孕酮炔雌醇对多次人工流产后宫腔粘连患者子宫内膜血管内皮生长因子表达的影响 被引量:7

摘要 目的:探讨左炔诺孕酮炔雌醇对多次人工流产后宫腔粘连患者的治疗作用及对子宫内膜血管内皮生长因子(VEGF)表达的影响。方法:选择2009年1月~2010年12月我院妇产科行多次人工流产而出现宫腔粘连的患者75例,宫腔粘连患者均进行宫腔镜手术。随机分为对照组37例,研究组38例,研究组在术后当晚开始服用左炔诺孕酮炔雌醇(三相)片治疗,持续3个月。采用免疫组织化学方法检测子宫内膜VEGF,比较两组治疗前、治疗后的变化。结果:两组均无重度粘连。研究组宫腔粘连发生率为7.89%,对照组宫腔粘连发生率为21.62%,差异有统计学意义(P<0.05)。与治疗前比较,对照组、研究组可见VEGF下降明显,差异有统计学意义(P<0.05)。与对照组比较,研究组VEGF明显减低,差异有统计学意义(P<0.05)。结论:左炔诺孕酮炔雌醇对多次人工流产后宫腔粘连患者子宫内膜VEGF有下调作用。 Objective: To discuss effect of levonorgestrel-ethinyl estradiol on expression of vascular endothelial growth factor(VEGF) of endometrium in patients with intrauterine adhesions due to recurrent abortion.Methods: A total of 75 cases with intrauterine adhesions due to recurrent abortion admitted from Jan 2009 to Dec 2010 were selected.They were randomly divided into control group(n=37) and study group(n=38).All patients received hysteroscope surgery.Patients in study group were orally administrated by levonorgestrel-ethinyl estradiol after surgery for 3 months.VEGF was detected by immunohistochemical method.And the VEGF levels in two groups were compared before and after surgery.Results: No severe adhesions occurred in two groups.The incidence of intrauterine adhesions was 7.89% in study group and 21.62% in control group,with significance difference between two groups(P〈0.05).VEGF were significantly decreased after treatment in two groups(P〈0.05).Compared with control group,the VEGF was significantly decreased in study group(P〈0.05).Conclusions: Levonorgestrel-ethinyl estradiol can down regulate VEGF of endometrium in patients with intrauterine adhesions due to recurrent abortion.
