摘要
目的观察低钙透析串联血液灌流治疗维持性血液透析患者肾性骨病的疗效。方法所有患者均使用Fresenius4008S透析机,每次透析4h,每周3次。对照组使用常规血液透析联合活性VitD3治疗,治疗组在活性VitD3治疗基础上,使用低钙透析串联血液灌流治疗。观察两组患者在治疗8周后皮肤瘙庠和骨痛情况及iPTH、血钙、血磷的变化。结果两组患者治疗前后血钙均无明显变化;治疗组治疗后血磷含量较治疗前有明显降低(P<0·05),治疗组治疗后血清全段甲状旁腺激素(iPTH)较治疗前显著性降低(P<0·01);治疗组患者经治疗后显效患者8例,进步10例,总有效率为90·0%,无恶化情况出现;对照组显效患者0例,进步7例,出现4例病情恶化患者。结论低钙透析能有效的控制患者的血钙水平,血液灌流能明显降低患者的血磷及iPTH含量,减轻并消除患者的症状,对减缓患者肾性骨病的发生和发展有积极作用。
Objective Observe low calcium dialysis the curative effectof the blood dialysis sufferer the kidney bone disease for establishingblood to infuse to flowto cure maintenance.Methods All sufferers all use Fresenius4008 Sdialysis machine,each time dialysis 4 hours,weekly 3 times.The matched control uses normal regulations blood dialysis to unite live vitD 3 treatments,The treatment set is in the live vitD3 cure foundation up,use lowcalcium dialysis establish blood to infuse to flowa treatment.Observe two sets of sufferers at cure after 8 weeks skin Sao Xiangand bone pain the circumstance and iPTH,blood calciumand blood Be phosphoric to change.Results Two sets of sufferers cure front and back blood calciumto all have no obvious variety;The treatment set cures blood Lin behind to compare with treatment,content ex-obviously lower（P0·05）,the iPTHmore shows Zhao to lower before curing after the treatment set treatment;（P0·01） The treatment set sufferer after treatment shows effect sufferer＇s 8 and progress 10,always efficient appear for 90·0%,having no worsen circumstance;The matched control shows effect sufferer＇s 0 and progress 7,appear 4 conditions to worsen a sufferer.Conclusion The low calcium dialysis ability is effective of the control sufferer＇s blood calcium level,the blood infuses to flowthe blood Lin and iPTHcontent that can obviously lower a sufferer and eases and get rid of the sufferer＇s symptom,have positive effect towards decelerating a sufferer kidney bone getting sick of occurrence and development.
