髂腹下神经-髂腹股沟神经阻滞复合全身麻醉在小儿腹股沟手术中的应用 被引量:7

摘要 目的观察髂腹下神经-髂腹股沟神经阻滞复合全身麻醉在小儿腹股沟区手术中的应用效果。方法 60例单侧腹股沟区手术患儿,随机分为A组和B组各30例,A组采用髂腹下神经-髂腹股沟神经阻滞复合全身麻醉,B组采用单纯全麻。观察2组入手术室时(T1)、手术切皮时(T2)及手术结束时(T3)各时点患儿的心率(HR)、血氧饱和度(SpO2)及平均动脉压(MAP),记录氯胺酮、咪达唑仑总用量及术后苏醒时间。结果 A组HR、MAP在各时点无明显改变,B组HR、MAP在T2、T3时点高于T1,差异均有统计学意义(P<0.05);A组HR、MAP在T2、T3时间点均低于B组,差异均有统计学意义(P<0.05);2组SpO2各时间点无明显变化,差异无统计学意义(P>0.05)。A组氯胺酮和咪达唑仑总用量少于B组,术后苏醒时间明显短于B组,差异均有统计学意义(P<0.05和P<0.01)。结论髂腹下神经-髂腹股沟神经阻滞复合全身麻醉,在较浅全麻下能产生良好的麻醉效果,减少全麻药用量,有利于患儿的术中管理和术后尽快苏醒,是小儿腹股沟区手术较为理想的麻醉方法。 Objective To observe the application effects of iliohypogastric nerve-ilioinguinal nerve block combined with general anesthesia in the children′s inguinalis surgery.Methods 60 pediatric patients undergoing hemi-inguinalis surgery were randomly divided into group A and group B,30 cases of each.Group A were given iliohypogastric nerve-ilioinguinal nerve block combined with general anesthesia,while Group B were given pure general anesthesia.To observe the patients′s heart rate(HR)、Pulse Oxygen Saturation(SpO2) and mean artery pressure(MAP) when they just came into the operation-room (T1),just the beginning operation (T2) and the finishing operation (T3),and record the amount of ketamine and midazolam, as well as the wake time after operation.Results The HR、MAP of group A were not considerable change at all time points,but the HR、MAP of of group B at T2 T3 were higher than that at T1,and the differences were statistically significant(P0.05).The HR、MAP of group A at T2、T3 were lower than that of group B ,and the differences were statistically significant(P0.05).There was no statistical significance between the SpO2 of 2 groups at all time points(P0.05).The amount of ketamine and midazolam of group A were considerably less than that of group B,and the wake time after operation were shorter than that of group B,and the differences were statistically significant(P0.05 and P0.01).Conclusion The iliohypogastric nerve-ilioinguinal nerve block combined with general anesthesia can take good anesthetic effect during light anaesthesia ,decrease the dosage of general anesthetic drugs and facilitate management of patients during operation,as well as wake quickly after operation, which is an ideal anesthesia method in the children′s inguinalis surgery.
