摘要
目的:比较盐酸小檗碱联合3种常用抗真菌药物伊曲康唑、特比萘芬和灰黄霉素对犬小孢子菌体外抗菌活性。方法:实验参照美国临床实验室标准化研究所(CLSI)颁布的M38-A2方案和相关文献对临床分离的30株犬小孢子菌进行体外药敏试验。其中盐酸小檗碱的最小抑菌浓度(MIC)值定义为与生长对照相比,肉眼观察出现100%生长抑制的最低药物浓度。采用分级抑制浓度指数(FICI)评定两药联合效果。FICI≤0.5,两药为协同作用,FICI>4,两药为拮抗作用,0.5<FICI≤4,两药为无关作用。结果:盐酸小檗碱和不同浓度灰黄霉素联合,以0.5 n/gL灰黄霉素和盐酸小檗碱联合FICI值≤0.5所占比例最高,协同作用最强,P<0.05,差异有统计学意义。盐酸小檗碱和不同浓度伊曲康唑联合,以0.125 ng/L和0.250 ng/L伊曲康唑和盐酸小檗碱联合,FICI≤0.5所占比例最高,P<0.05,差异有统计学意义。盐酸小檗碱和不同浓度特比萘芬联合,以0.04 ng/L特比萘芬和盐酸小檗碱联合,FICI≤0.5所占比例最高,但P>0.05,差异无统计学意义。盐酸小檗碱分别与灰黄霉素、伊曲康唑、特比萘芬联合FICI值≤0.5所占比例分别为25.00%、40.38%和18.93%,3种联合方案以盐酸小檗碱和伊曲康唑联合协同作用最强,P<0.05,差异有统计学意义。结论:盐酸小檗碱联合应用灰黄霉素、特比萘芬、伊曲康唑在体外均有不同程度协同抗犬小孢子菌活性作用,以盐酸小檗碱联合伊曲康唑协同作用最强。
Objective: To compare the activities of berberine hydrochloride combined with three kinds of commonly used antifungal agents itraconazole, terbinafine and griseofulvin against Microsporum canis in vitro. Methods: According to the pro- gram of M38-A2 from the United States National Clinical and Laboratory Standards Institute (CLSI) and relevant literatures, susceptibility test was done with 30 clinical isolates of Microsporum canis in vitro. The MIC value of berberine was defined as the 100% growth inhibition of the minimum drug concentration that can be noticed with naked eyes compared with the growth control. Fractional inhibitory concentration index (FICI) assessed the joint effect of the two drugs. FICI≤0.5, the two drugs for synergy; FICI〉4, the two drugs for antagonism; 0.5〈FICI≤4, the two drugs no interaction. Results: In the combination of berberine hydrochloride and griseofulvin, 0.5 ng/L griseofulvin had the strongest anti-fungal effects, with statistically significant difference, (P 〈 0.05). The same situations were also found in the combinations of berberine hydrochloride with itraconazole (0.125 ng/L and 0.25 ng/L) or with terbinafine (0.04 ng/L ), the joint synergy was the strongest (P〉 0.05), but the difference was no statistically significant. The percentage of FICI≤0.5 was 25.00%, 40.38% and 18.93% in the combination of berberine hydrochloride with griseofulvin, itraconazole and terbinafine respectively, with the strongest joint synergy for itra- conazole combination (P〈 0.05). Conclusions: Berberine combined with griseofulvin, terbinafine, or itraconazole could have the synergistic activity against Microsporum canis, with best synergistic activity of berberine and itraconazole.
