药学服务对骨科Ⅰ类切口手术预防用抗菌药物干预效果研究 被引量:17

Effects of pharmaceutical service intervention on antibacterial use in the first category of orthopedic surgery

摘要 目的探索药学服务对骨科Ⅰ类切口手术预防用抗菌药物的干预途径,规范某三级甲等医院骨科I类切口手术围术期抗菌药物预防性应用与管理。方法整群选取骨科I类切口手术2007年6至12月的出院病例为干预前组,2009年6至12月的出院病例为干预后组,研究干预前后预防性应用抗菌药物合理性。结果抗菌药物品种选择不合理率由干预前组51.15%下降为干预后组44.53%,术后抗菌药物的使用平均时间由干预前组的6.82 d下降为干预后组的5.67 d,术前预防用抗菌药物时机不当由干预前组的81.61%下降至预后组的7.03%(P<0.01),围术期抗菌药物的不合理联用由干预前组的23.36%下降至干预后组的7.03%(P<0.05)。实施药学服务的干预后组较干预前组各项不合理用药率均有不同程度的下降。结论恰当的药学服务措施能有效提高医院I类切口手术围术期预防性应用抗菌药物的合理水平。 Objective To regulate the prophylactic application and management of antibacterials during the perioperative period in the first category of orthopedic surgical incision in a hospital,to explore the pharmaceutical service intervention in antibacterial use in the first category of orthopedics surgery.Methods The discharged cases of the first category of orthopedic surgery from June to December 2007 were selected as the control group.The discharged cases of the first category of orthopedic surgery from June to December 2009 were selected as the intervention group.The intervention effects of pharmaceutical services on using antibacterials between the 2 groups were compared.Results The irrational selection of antibacterials was decreased in the intervention group from 51.15% to 44.53%,the average days of each orthopedic surgery patient using antibacterials decreased in the intervention group from 6.82 days to 5.67 days,the rate of improper use of antibacterials before the operation decreased in the intervention group decreased from 81.61% to 7.03%(P〈0.01),and the rate of improper combination of antibacterials in the perioperative period in the surgical intervention group decreased from 23.36% to 7.03%(P〈0.05).Conclusion Suitable pharmaceutical service can improve the reasonable use of antibacterials in the perioperative period.
