摘要
目的:探讨腰椎间盘突出症(LDH)合并软组织损伤的红外热像(ITI)表现。方法:选择符合LDH诊断的患者55例,运用红外热像仪采集其下部躯干及下肢的ITI进行对比分析。结果:55例的ITI腰骶部均表现为腰椎部菱形区高温区增大,呈长条形红色或深红色片状高温或小团状高温,与突出侧符合。其中26例(47.3%)并发腰肌筋膜炎者表现为腰椎旁片状高温向旁边延伸。15例(27.3%)的患肢ITI表现LDH标准图谱,为患肢异常低温区与突出侧神经支配皮肤区符合。40例(72.7%)的患肢温度没有显著降低,其中19例温度升高与下降同时并存(34.5%),12例温度接近正常(21.8%),9例温度升高(16.4%)。诊断为患肢合并肌筋膜疼痛综合征(MPS)24例(43.6%),合并骨关节炎(OA)5例(9.1%),有MPS与OA并存6例(10.9%),静脉曲张1例(1.8%),皮肤陈旧性疤痕2例(3.6%),原因不明2例(3.6%)。结论:LDH患者合并有腰部软组织损害性病变时,ITI出现不典型红外热图像。
Objective: To explore infrared thermal imaging(ITI) findings of lumbar disc herniation(LDH) combined with soft tissue injury.Methods: 55 cases of LDH were examined with infrared thermography,and their ITI of waist below body was contrastively analyzed.Results: There was increased high temperature along the lumbar diamond on 55 cases of ITI,showing a long mass of red or deep red foliated high temperature or a small group high temperature,corresponding with extrusive lateral.There was a sheet of high temperature in lumbar paraspinal region extending sidewise on 26 cases(47.3%) combined with local psoas myofascitis.The ITI of affected lower limb on 15 cases(27.3%) was presented standard imaging of LDH,showing that abnormally low temperature region of affected lower limb was coinciding with the innervation skin area of extrusive lateral.The temperature of affected lower limb on 40 cases(72.7%) didn't significantly decrease,and it was the simultaneous presence of increment and drop in 19 patients(34.5%),approximately normal in 12 patients(21.8%) and warmer in 9 patients(16.4%).The affected lower limb was diagnosed as MPS in 24 patients(43.6%),osteoarthritis in 5 patients(9.1%),the presence of MPS and osteoarthritis in 6 patients(10.9%),phlebeurysma in 1 patient(1.8%),some old scars in 2 patients(3.6%) and unknown reason in 2 patients(3.6%).Conclusion: ITI is not a typical image in LDH patient with tumbar soft tissue lesion.
