摘要
目的探讨急性等容血液稀释(ANH)联合控制性降压(CH)对减少全髋置换患者术中失血的效果。方法将68例全髋置换患者随机均分为实验组和对照组。实验组于全麻诱导后采用ANH和CH;对照组麻醉后常规输注复方乳酸钠。比较两组术中失血量、输血量、尿量以及术前、术毕、术后第1、7天的Hb和Hct值。结果实验组术中失血量、输血量显著少于对照组,尿量显著多于对照组(均P<0.01);两组术后不同时段Hb和Hct值显著低于术前(均P<0.01),但组间比较,差异无统计学意义(均P>0.05)。结论 ANH联合CH能减少患者术中出血和输血,且对Hb和Hct无显著影响。
Objective To investigate the effect of acute normolvolemic hemodilution（ANH） combined with controlled hypotension（CH） in the replacement of total hip joint to decrease blood lose.MethodsSixty-eight patients of the replacement of total hip joint were randomly divided into two groups： an experimental group and a control group.After general anaesthesia,ANH and CH were employed in the experimental group.The patients in the control group were transfused routinely compound sodium lactate the blood loss,blood transfusion voulume,and the urine volume were recorded.The Hb and Hct were tested on the 1st day and the 7th day after the operation.ResultsThe blood loss and the blood transfution volume in the experimental group were significantly lower than those of the control group（P〈0.01 for both）;the urine volume in the experemental group was higher than that of the control group（P〈0.01）.After the operation,the Hb and Hct were all lower than those before the operation at different time points（P〈0.01 for both）.But there are no significant differences between the two groups（P〈0.05）.ConclusionAHH combined with CH may reduce blood loss and blood transfusion volume and has no conspicuous effect on Hb and Hct.
