摘要
目的探讨尿毒症患者高瘦素血症与微炎症状态的关系。方法选择我院泌尿外科透析中心尿毒症患者73例为尿毒症组,另外选择同期体检中心健康体检者30名为对照组。比较2组体质量指数(BMT)、血肌酐(SCr)、尿素氮(BUN)、血清瘦素、肿瘤坏死因子α(TNF-α)、白细胞介素(IL)-6、IL-10及外周血中性粒细胞吞噬功能。结果尿毒症组BMI、IL-10均低于对照组,SCr、BUN、血清瘦素、TNF-α、IL-6均高于对照组(P〈0.05),尿毒症患者外周血中性粒细胞吞噬率低于对照组(P〈0.05)。经单因素相关分析显示,尿毒症组血清瘦素水平与TNF-α、IL-6均呈正相关(r分别为0.58、1.00,P〈0.05),与IL-10呈负相关(r=-0.45,P〈0.05)。结论尿毒症患者高瘦素血症及其相关的微炎症反应共同参与了疾病的发生与发展,且瘦素是重要的炎症调节因子。
Objective To explore the relationship between high leptin and mieroinflammatory state in uremic patients. Metlmds A group of 73 uremic patients in dialysis centre of our department was assigned as uremic group, and a group of 30 health persons who were examined over the same period served as normal group. The body mass index (BMI), serum creatinine (SCr), blood urea nitrogen (BUN), leptin, tumor necrosis factor-alpha (TNF-α), interleukin (IL)-6, IL-10 and the neutrophils phagoeytosis function were compared between two groups. Results BMI and IL-10 were lower, and the levels of SCr, BUN, leptin, TNF-α, IL-6 were higher in uremic group than in normal group (P〈0. 05). The neutrophils phagocytosis function in uremic group was significantly decreased as compared with normal group (P〈0. 05). One-way ANOVA analysis revealed that serum leptin was positively correlated with the levels of TNF-α and IL-6 (r= 0. 58 and 1.00,P〈0. 05), but negatively with the levels of IL-10 (r= -0. 45, P〈0. 05). Conclusions The high level of leptin and the related microinflarnmation are involved in the initiation and development of uremia, moreover leptin is an important mediator.
