心力衰竭并肺部感染患者血浆脑钠肽、血浆TNF-α及IL-6变化及其与心功能的关系 被引量:16

摘要 目的探讨心力衰竭合并肺部感染的患者血浆脑钠肽(BNP)、TNF-α及IL-6变化及其与心功能的关系。方法选择2008年1月至2009年12月我院收治的心力衰竭合并肺部感染的患者60例作为研究对象,按照患者心功能分级NYHA分为Ⅰ级12例,Ⅱ级22例,Ⅲ级14例,Ⅳ级12例,并随机选取本院体检中心健康志愿者20例作为对照者。采用酶联免疫吸附法(ELISA)检测检测者血清白介素6(IL-6)和肿瘤坏死因子α(TNF-α)及血浆脑钠肽(BNP)水平。结果①研究组与对照组IL-6及TNF-α比较差异无统计学意义(P>0.05)。②研究组血浆BNP水平显著高于对照组血浆BNP水平(P<0.01)。③研究组60例患者随心功能下降血浆BNP水平逐渐增高(P<0.01)。结论血浆脑钠肽水平能较好的反映心力衰竭患者心功能水平,对于心力衰竭合并肺部感染的患者诊断及病情评估具有重要的临床价值。 Objective To discuss the change of plasma brain natriuretic peptide,plasma TNF-α and IL-6 in heart failure patients with pulmonary infection and its relationship with cardiac function.Methods 60 cases with the combination of heart failure and pulmonary infection from Jan.2008 to Dec.2009 were divided into grade Ⅰ(n=12),grade Ⅱ(n=22),grade Ⅲ(n=14),and grade Ⅳ(n=12).While 20 healthy volunteers were chosen as control group.The level of IL-6,TNF-α and plasma brain natriuretic peptide were detected with ELISA.Results The level of IL-6 and TNF-α had no obvious difference in study group and control group.The level of plasma brain natriuretic peptide in study group was significantly higher than that in control group(P0.01).In study group,with the reduction of cardiac function,the level of plasma brain natriuretic peptide was gradually increased(P0.01).Conclusion The level of plasma brain natriuretic peptide can better reflect the cardiac function level of heart failure patients.The diagnosis and severity evaluation are of important clinical value.
