摘要
目的为进一步探讨了解喉癌手术的治疗方法和疗效,以改进治疗方法、提高治疗效果。方法对208例喉癌患者进行手术治疗,其中声门上型53例,声门型150例,声门下型5例,属于Ⅰ期的29例,Ⅱ期的64例,Ⅲ期的75例,Ⅳ期的40例,根据每位患者的病变部位情况不同,各采用相应的手术方式,观察其临床疗效。结果所有的患者恢复后以口进食,喉部分切除患者的发音均恢复。喉部分切除的有173例,占全部病例的83.2,喉部分切除术拔管的有176例,占84.6。Ⅲ、Ⅳ期的患者,喉部分切除的有115例,占55.3。全部的喉癌患者1年生存率为92,3年生存率为86.5,5年生存率为67.7,Ⅲ、Ⅳ期的喉部分切除术和全喉切除术患者的生存率无统计学差异(P>0.05)。结论喉癌手术治疗效果良好,可以保留喉的全功能,但喉癌发病期和发病部位的不同造成情况各异,临床治疗时还要制定个体化的治疗方案,运用合适的手术方法,提高治疗效果。
Objective Laryngeal surgery for further study to understand the treatments and curative effect,to improve the therapeutic methods,improve therapeutic effect.Methods 208 cases of laryngeal cancer patients,including laryngeal surgery on 53 patients subglottic type,type 150 cases,subglottic 5 cases,Ⅰ period 29 cases,Ⅱ period 64 cases,Ⅲ period 75 cases,Ⅳ period 40 cases,according to each situation is different,the lesion site using the corresponding operation method,observed the clinical curative effect.Results All the patients in the mouth after the restoration of eating and throat resection patients were restored the pronunciation of.Throat partial nephrectomy 173 with an example,the total cases,throat 83.2%of partial nephrectomy decannulation obligatory examples,occupies 176（84.6%）.Ⅲ,Ⅳ period of patients,larynx partial nephrectomy,accounting for the had 115 patients 55.3%.All of the laryngeal cancer patients 1-year survival was 92%,3-year survival for 86.5%,the 5-year survival rate for Ⅲ,Ⅳ period,67.7% resection of the throat and whole al.pharyngocutancous fistu-la survival was not statistically significant（P0.05）.Conclusion Laryngeal cancer surgery effect is good,can keep all the functionality,but throat laryngeal period of disease and come on at different features,clinical treatment caused even setting indiidualized treatment plan,the appropriate surgical method,improve the therapeutic effect.
