未接受蛋白酶抑制剂治疗者的HIV-1基因的多态性及蛋白酶抑制剂耐药相关性变异

Protease gene polymorphism and drug resistance related variation in HIV-1 infected patients having not received protease inhibitor therapy

摘要 目的:研究未接受蛋白酶抑制剂(PIs)治疗的人类获得性免疫缺陷病毒1(HIV-1)感染者HIV-1多态性以及PIs耐药相关性变异的分布情况。方法:取自中国4个省市(上海、安徽、河南和云南)81例未接受PIs治疗的HIV-1感染者中分离的病毒株,经反转录聚合酶链反应(RT-PCR)扩增HIV-1病毒的pol基因,并进行测序及进化分析。结果:HIV-1病毒株基因最多的为B′亚型(82.7%),其他分别为CRF01_AE亚型(11%)、CRF07_BC或CRF08_BC亚型(3.7%)、CRF02_AG亚型(1.2%)以及B亚型(1.2%)。B′亚型主要存在于有偿助血员以及经输血感染的患者中。R41K、K70Q/R/E和L89M变异在非B亚型中比在B′和B亚型中更为常见。次要PIs耐药相关性变异I15M/V、K20M/R/I、M36I和H69K在非B亚型中比在B和B′亚型中更为常见。相对于非B亚型,次要PIs耐药相关性变异I62V、L63P、A71V/T、V77I和I93L则在B和B′亚型中更为常见。在3例(3.7%)感染B亚型的患者中发现了主要PIs耐药相关性变异M46I。结论:在未接受PIs治疗的患者中,HIV-1病毒基因多态性及次要PIs耐药相关性变异的出现率较高,而主要PIs耐药相关性变异的流行率则较低。 Objective To investigate the polymorphism of HIV-1 and the distribution of protease inhibitor resistance-associated diversity in human immunodeficiency virus-1(HIV-1) infected patients having not received protease inhibitor therapy.Methods HIV virus strains were taken from eighty-one HIV-1 infected patients having not received protease inhibitor therapy from four provinces/municipality(Shanghai,Anhui,Henan and Yunan) in China.Reverse transcription polymerase chain reaction was used to amplify the pol gene of HIV-1 and then it was sequenced and analyzed phylogenetically.Results The most prevalent HIV-1 was subtype B′(82.7%),followed by CRF01_AE(11%),CRF07_BC or CRF08_BC(3.7%),CRF02_AG(1.2%) and B(1.2%).The B′ subtype was mainly found in paid plasma blood donors and blood transfusion infected patients.Polymorphisms R41K,K70Q/R/E and L89M were more frequently found in non-B subtypes than in B/B′ subtypes.Minor drug resistance related variation I15M/V,K20M/R/I,M36I,H69K were more frequently found in non-B subtypes than in B′/B subtypes.However,minor drug resistance related variation I62V,L63P,A71V/T,V77I,I93L were more frequent in B/B′ subtypes than in non-B subtypes.Major drug resistance related variation M46I was found in 3 patients(3.7%) infected with B subtypes.Conclusions In HIV-1 infected patients having not received protease inhibitor therapy,the prevalences of gene polymorphism and minor drug resistance related variation are quite high while the prevalence of major drug resistance related variation is low.
