摘要
目的探讨手术治疗移位肱骨小头骨折的方法及疗效。方法肘关节外侧入路手术治疗移位肱骨小头骨折13例,Ⅱ型2例行骨折片切除,其余病例行克氏针螺钉内固定。结果术后伤口Ⅰ期愈合,无神经、血管损伤。X线显示骨折解剖复位。骨折愈合4~8周,平均6周。根据改良Cassebaum评分系统进行功能评价,优8例,良4例,可1例,优良率92.3%。结论肱骨小头骨折为囊内骨折,应采用切开复位内固定治疗,对于骨折片极小,无法固定者可行骨折碎片切除术。术后早期功能锻炼对于肘关节功能恢复有促进作用。
Objective To explore the surgical treatment of humerus fracture displacement small head method and curative effect.MethodsElbow lateral approaches surgery shift humerus fractures in 13,small head Ⅱ type 2 routine piece resection,the remaining cases fractures do gram needle screw internal fixation.ResultsPostoperative wound stage I healed without nerve,vascular injury.X-ray display fracture anatomical reattachment.Fracture healing 4 to 8 weeks,and the average six weeks.According to the improvement Cassebaum scoring system for functional evaluation,optimal 8 cases ＂good＂ in 4 cases,can be in 1,dykes 92.3 percent.ConclusionThe humerus fractures small head for sac fractures,should adopt open reduction and internal fixation for fracture,for fracture piece is tiny,cannot the feasible fixed fracture fragments resection.Early postoperative function exercise for elbow functional recovery declination.
