摘要
目的观察结膜下注入不同基质眼膏后,组织的炎症反应、病理变化及眼膏的降解吸收情况。方法将72只豚鼠随机分为3组,以凡士林和液状石蜡为基质的红霉素眼膏为A组,以玻璃酸钠和卡波姆为基质的氧氟沙星眼膏为B组,C组结膜下注射甲基纤维素作为对照组。3种药物各0.2mL分别注入豚鼠上方球结膜下,于注射后1d、3d、7d、14d、21d、28d,6个时间点观察3组结膜充血肿胀、眼膏降解与扩散及组织病理学改变。结果 A组结膜肿胀明显,眼膏液化向周围间隙扩散,未见明显降解吸收;病理变化显示早期炎性细胞以中性粒细胞为主,晚期纤维组织增生机化。B组结膜充血较明显,眼膏呈斑驳状扩散并逐渐被降解吸收;炎性细胞数目多于A组,但仅少量纤维组织增生。C组结膜肿胀轻、充血不明显;炎性细胞浸润少。结论 A组眼膏在结膜下反应最重、降解吸收最慢、病理变化最明显,建议不要用于有创口的组织表面;B组眼膏可降解吸收、增生较轻,认为可用于泪道探通术后和眶内填充。
Objective To observe the inflammatory reaction,degradation and absorption,histopathological changes of different substrate eye ointments entering the subconjunctival tissues.Methods Seventy-two guinea pigs were divided into three groups randomly,the vasline and liquid paraffin were set as substrate of erythromycin eye ointment in group A,the sodium hyaluronate and carbomer were set as substrate of ofloxacin eye ointment in group B,and the methylcellulose was subconjunctival injected in group C as control.The three medicines were injected into the upper subconjunctival tissue of guinea pigs in 0.2 mL,the congestion and swelling of conjunctiva,the degradation of ointment and the local histopathological changes at 1 day,3 days,7 days,14 days,21 days and 28 days after injection in three groups were observed.Results The swelling of conjunctiva in group A was obvious;The eye ointment was liquefied and diffused to the around space,degradation and absorption were not obviously seen;The histopathological changes showed that the neutrophils were the main inflammatory cells in early time and the fiber tissue were formed in later time.The congestion of group B was relatively obvious;The eye ointment spreaded in shape of spot and was gradually degraded and absorbed;The number of inflammatory cells were more than that in group A,but there were little fibroplasias.The swelling of conjunctiva was lighter and the congestion was not obvious in group C;The inflammatory cells were few.Conclusions The reaction of eye ointment in group A is the most serious,the degradation and absorption are the most slowest,and the histopathological changes are the most obvious,therefore,it is not recommended to be used on the surface of wound;The eye ointment in group B can be degraded and absorbed and has little fibroplasias,so it could be used in the lacrimal duct probing surgery and orbit supplement.
