摘要
目的探讨经尿道电切术治疗超大前列腺增生的临床效果。方法回顾性分析838例前列腺增生患者行TURP治疗的临床资料,79例前列腺重量大于100克的被分为第1组,759例前列腺重量小于100克的被分为第2组。术前参数(前列腺重量,IPSS评分,最大尿流率,残余尿量)和术后资料(手术时间,切除组织重量,并发症)被记录。结果术后6月随访,两组患者术后IPSS评分,最大尿流率,残余尿量均较术前明显改善。第1组患者手术时间长于第2组,但两组患者并发症的发生率类似。术后与第2组相比,第1组在IPSS评分和最大的尿流率方面有更多改善。结论 TURP治疗超大前列腺增生是安全、有效的。
Objective To research the clinical effect of TURP in the treatment of benign prostate hyperplasia between 〉100 gram prostate gland. Methods We prospectively evaluated 838 patients treated with TURP for BPH.A total of 79 patients with a prostate weight of more than 100 gram were assigned to group 1.,while 759 patients with prostate weight less than 100 gram were assigned to group 2.Preoperative parameters were considered in each patient,including prostate weight,IPSS,MFR,PVR.Operative time, resected tissue weight and all complications were recorded.All patients were evaluated 6 months postoperatively. Results Each group achieved significant improvement in IPSS,MFR and PVR after operation.Operative time was significantly longer in group 1 but the complition rate was similar in the 2 groups. Group 1 resulted in better improvements in MFR and IPSS. Conclusions TURP for huge prostate glands is a safe and effective procedure without showing a different complication rate compared with TURP for less than 100 gram prostate gland.
