摘要
目的探讨海洛因依赖者心理健康状况与防御方式的相关性,为制定治疗策略提供依据。方法对180例海洛因依赖者采用自拟一般资料调查表、症状自评量表和防御方式问卷进行测评分析。结果海洛因依赖者症状自评量表总分及各因子分均显著高于国内常模(P〈0.01);各因子阳性症状检出率依次为躯体化(62.2%)、抑郁(58.3%)、强迫(57.2%)、敌对(49.4%)、焦虑(48.3%)、偏执(45.0%)、人际关系(43.9%)、精神病性(41.1%)、恐怖(26.1%);女性防御方式问卷掩饰因子评分及幻想、升华、制止、回避、同一化、消耗倾向项目评分显著高于男性(P〈O.01);防御方式问卷不成熟防御机制、中间型防御机制因子评分和投射、被动攻击、潜意显现、抱怨、退缩、躯体化、消耗倾向项目评分与症状自评量表总分及各因子分均呈显著正相关(P〈0.05或o.01),幻想、压抑、假性利他项目评分与症状自评量表总分及各因子分无显著相关(P〉O.05),其余因子及项目评分与症状自评量表总分或部分因子分呈显著正相关(P〈O.05或o.01)。结论海洛因依赖者心理健康水平较低,与其防御机制倾向密切相关。
Objective To explore the correlation between mental health status and defense style in heroin addicts in order to provide basis for instituting a regimen of therapy. Methods Assessments were carried out in 180 heroin addicts with the Self-made General information Questionnaire (SGIQ), the Symptom Checklist-90 (SCL-90) and the Defense Style Questionnaire (DSQ). Results The total and each factor score of the SCL-90 were all significantly higher in the addicts compared with national norm (P〈0.01) the detection rate of positive symptoms in each factor were somatization (62.2%), depression (58, 3%), obsessive-compulsive disovder (57.20%), hostility (49.4%), anxiety (48.3%), paranoia (45.0%), interpersonal sensitiveness (57.2%), psychotic (41. 1%) and phobia (26. 1%). L score and such items' scores were significantly higher in female subjects than in the male as fantasy, sublimation, undoing, avoidance, identification and consumption (P〈0.05 or 0.01); scores on immature and neurotic defense and on such items as projection, passive aggression, acting out, complaint, regression, somatization and con- sumption were significantly positively related to the toatl and each factor score of the SCL-90 (P〈0.05 or 0.01), scores on fantasy, suppression and pseudo-altruism were not to (P〉0.05) , and scores on the other factors and items were to the total and partial factor scores (P〈0.05 or 0.01). Conclusion The mental health level of heroin addicts is Door and is closely related to defense mechanisms.
