摘要
目的铁是人体必需微量元素,铁元素的缺乏不仅会导致贫血,还会影响幼儿的行为及智力发育。文中旨在分析不同胎龄新生儿静脉血清铁蛋白(serum ferritin,SF)、血清(serum iron,SI)及末梢血铁含量的差异。方法采集167例新生儿出生后统一检测第1周静脉血及末梢血常规,SF采用酶结合免疫吸附测定(enzyme-linked immunosorbent assay,ELISA)法测定,SI采用电感偶合等离子体原子发射光谱ICP-AES法测定,末梢血常规用日本Sysmex CE-2100血球分析仪测定,并对结果进行分析;根据不同胎龄进行数据比较并总结特点。结果不同胎龄新生儿SF、SI差异有统计学意义,早产组的外周血血红蛋白(hemoglobin,Hb)、红细胞计数(red blood cell count,RBC)、红细胞压积(hematocrit,HCT)水平较足月组明显降低(P<0.05),体重与SI呈正相关,Hb与SF、SI无相关关系,红细胞分布宽度(red blood cell distribution width,RDW)与SF、SI呈负相关。结论不同胎龄新生儿铁元素含量各有特点,应注意检测和分析以预防缺乏铁元素而导致对儿童生长发育的影响。
Objective The deficiency of iron,an essential trace element in the human body,may not only cause anemia in children but also affect their behavioral and mental development.The authors analyzed the differences in the concentrations of serum ferritin(SF) and serum iron(SI) in the venous blood of neonates of different gestational ages.Methods We collected the venous blood samples from 167 neonates within the first week after birth,determined the content of SF by ELISA and that of SI by inductively coupled plasma atomic emission spectrometry(ICP-AES),and analyzed the results of peripheral blood routine examinations obtained with the Japanese Sysmex CE-2100 blood analyzer.Then we compared the data and characteristics according to different gestational ages of the neonates.Results Statistically significant differences were found in SF and SI among the neonates of different gestational ages.The preterm group showed remarkably lower levels of peripheral blood hemoglobin(Hb),red blood cell count(RBC) and hematocrit(HCT) than the full-term group(P0.05).The body mass was positively correlated with SI,the red blood cell distribution width(RDW) negatively with SF and SI,while Hb with neither SF nor SI.Conclusion The content of serum ferritin varies in neonates of different gestational ages.Attention should be paid to its determination and analysis in order to prevent neonates from the influence of iron deficiency on their development.
