摘要
目的:观察兔部分肝脏缺血/再灌注损伤(I/R)磁共振弥散加权成像(MR-DWI)表面扩散系数(ADC)的演变规律,分析其与肝酶(ALT、ALP)相关性。方法:新西兰大白兔阻断肝左叶血供60min后,恢复血供,按再灌注时间分为6h、12h I/R组及假手术对照组即sham组(每组6只,共计18只),DWI成像b值分别选取20s/mm2、50s/mm2、100s/mm2、300s/mm2和600s/mm2,同时行T2WI、T1WI扫描、组织病理学和肝生化AST、ALT检查。结果:各I/R组ADC低于sham组,其中在b=20s/mm2、50s/mm2、100s/mm2时,6h组ADC明显低于sham组(P<0.05)。在b=300s/mm2、600s/mm2时,6h、12h I/R组ADC与sham组之间均无统计学差异(P>0.05)。各I/R血清ALT、ALP明显高于sham组(P<0.05)。b=20s/mm2、50s/mm2、100s/mm2时,ALT与ADC存在显著负相关(r=-0.497,P<0.05;r=-0.623,P<0.05;r=-0.671,P<0.01);b=20s/mm2、100s/mm2,ADC与分别与ALP也存在相关性(r=-0.578,P<0.05;r=-0.489,P<0.05)。结论:采用较小b值弥散成像(DWI)能够动态监测肝I/R病理发展过程,ADC值可以作为检测肝脏损伤的敏感指标。
Objective: To obverse apparent diffusion coefficient(ADC) changes and correlation of serum amylase(ALT,ALP) in a rabbit hepatic ischemia reperfusion injury(I/R) model using magnetic resonance diffusion weighted imaging(MRDWI).Methods: After induction of hepatic I/R by left liver lobe ischemia(60 min) followed by 6 h and 12 h reperfusion,a total of 18 rabbits(one sham group and 2 I/R groups;n=6,for each) were imaged with a 3.0 T MRI protocol to obtain T2-weighted(T2WI),T1-weighted(T1WI) and diffusion-weighted imaging(DWI) with ADC maps using b values of 20,50,100,300 and 600 s/mm2.Blood samples were taken from the superior mesenteric vein to measure the levels of serum amylase(ALT,ALP)at different time point,and then liver samples were underwent histological examination after scans of every group were completed.Results: The ADC of I/R groups was lower than the sham group.ADC of 6 h group were significantly lower than that of sham group(P0.05) when b=20,50 and 100 s/mm2.There were clear differences between sham group and 6 h,12 h I/R group when b value was 300 and 600 s/mm2(P0.05),respectively.The levels of ALP and ALT in I/R groups were higher than those in the sham group(P0.05).When b values of 20,50 and 100 s/mm2 were used,ADC was closely correlated with ALT(r=-0.497,P0.05;r=-0.623,P0.05;r=-0.671,P0.01).While b=20 and 100 s/mm2 the difference of ADC was closely related to the increase of ALP with correlation coefficients of-0.578,and0.489,respectively(P0.05).Conclusion: DWI is sensitive to characterize the histopathologic changes with smaller b values and ADC may be a useful parameter in assessing progress of hepatic I/R.
