摘要
患者男,73岁。左侧臀部及右足底部出现多发性红色皮下结节及斑块,伴发热。曾被误诊为"血管炎"及"结节性脂膜炎"。皮损组织病理示:肿瘤浸润皮下脂肪组织,真皮深层及皮下脂肪层小叶间隔及小叶内大量淋巴细胞及组织细胞浸润,呈团块状,见散在异型淋巴细胞。免疫组化示:CD3(+),CD45RO(+),CD30(+),CD8(+),CD20(-)和CD4(-)。患者于化疗半月后因继发严重肺部感染而死亡。诊断:皮下脂膜炎样T细胞淋巴瘤。本文对本病进行相关的文献复习。
This case reported is a 73-year-old male, who had presented with multiple erythematous plaques on left buttock and right foot with fever. This patient has been misdiagnosed as vasculitis and nodular pannieulitis. The histopathologic findings showed that both deep dermis and subcutaneous fat was infiltrated with lots of lympboeytes and histioeytes, in which there are interspersed atypical lymphoeytes. Immunohistoehemieal studies showed CD3 （ ＋ ）, CD45RO （ ＋ ）, CD30 （ ＋ ）, CD8 （ ＋ ）, CD20 （ - ） and CD4 （ - ）. Unfortunately, this patient died from serious lung infection after combined chemotherapy. Diagnose： Subcutaneous panniculitis-like T cell lymphoma （SPTCL）. Furthermore, literatures about SPTCL were also reviewed.
