摘要
目的探讨定量组织速度成像(QTVI)评价不同PTH水平维持性血液透析患者的心肌功能。方法应用QTVI技术对20例健康人及40例尿毒症患者分成PTH<正常值上限3倍,PTH>正常值上限3倍两组,每组各20例,分别进行不同节段心肌运动速度测量,即收缩期S波,舒张期E、A波,同时测定其相应的时间-速度积分(S-TV I)位移的大小。结果 QTVI显示不同PTH水平维持性血液透析患者的心肌功能较正常人减低。两组患者E波、S及S-TV I均低于正常组的相应心肌节段,差异有显著意义P<0.05。PTH>正常值上限3倍的尿毒症患者心肌功能的减退程度更为严重。结论 QTVI技术能够早期、客观、准确的发现患者心肌细胞损伤的变化,为PTH对心肌的损害提供了客观、定量的理论依据。
Objective Quantitative tissue velocity imaging(QTVI) was used to detect themotion of different segmental cardiacmuscles and to analyze myocardial function of uremia patients with different quantity of PTH.Methods QTVI were performed in 40 patients with uremia and 20 healthy controls.40 patients diagnosis uremia were divided into 2 groups.20 patients with PTH less than 3 times of up normal limit,20 patients with PTH more than 3 times of normal limit.The regional velocities of systolic early diastolic and later diastolic period time velocity integration of S wave S-TV I were measured in the mid and basal segments.Results QTVI analyze myocardial function of uremia patients with different quantity of PTH had significantly decreased than healthy controls.Compared with healthy controls uremia group had significantly decreased E,S and S-TVI(P 0.05).20 uremia patients with PTH more than 3 times of normal limit were significantly reduced.Conclusions QTVI is another viable objective tool to quantitatively assess cardiacmuscles damaged by different quantity of PTH.It is proved that myoeardium is damaged by PTH with varying degree.
