摘要
目的通过回顾性分析,对视可尼喉镜健忘镇痛慢诱导气管插管的学习效率进行评价。方法 60例择期手术患者,年龄22~69岁,ASAⅠ~Ⅱ级,MallamptiⅠ~Ⅱ级,按学习的先后顺序分为三组,每组20例,记录插管试插例次,气管插管时间,同时记录患者麻醉诱导前、插管前即刻、插管后即刻、插管后1min的血压、心率、脉搏氧饱和度等生命体征的变化。术后随访气管插管的相关并发症。结果三组年龄、身高、体重、体重指数等一般情况无显著差异。三组平均插管时间分别为(66.8±18.4)s、(55.1±14.0)s、(48.6±11.2)s,第3组与第1组相比差异有统计学意义。一次插管成功率和两次总插管成功率第2、3组与第1组相比有了明显的提高。插管后即刻心率第3组与第1、2组相比有显著降低。第3组插管后1min血压和RPP与基础值相比有所降低,第2组插管后即刻心率与基础值相比有所增加,结果有差异性。所有患者术后均未发现气管插管相关并发症。结论随着学习经验的不断积累,视可尼喉镜健忘镇痛慢诱导气管插管插管时间显著缩短,一次插管成功率增高,并可减轻视可尼喉镜气管插管时对咽喉部的刺激反应。
Objective To evaluate the learning efficiency of amnesia and analgesia induction method to intubate by using shikani optical stylet.Methods 30 patients,aged from 22-69 years,ASAⅠ-Ⅱ,MallamptiⅠ-Ⅱ,were going to have elective surgeries were sequenced into three equal groups.The attempt times,intubating time were gained from the monitoring machines.And the vital signs,such as blood pressure,heart rate,and SpO2 changes,were recorded at four time point:before induction,instant time before intubation,instant time after intubation and 1min after intubation.Results It was comparable in terms of age,weight,height,body mass index(BMI).The average time of intubation in three group were 66.8±18.4 s,55.1±14.0 s,48.6±11.2 s respectively,and the time in group 3 was significantly shorter than that in group1.Successful rates of the first attempt and the second attempt to intubate in groups 2 and 3 were significant higher than that of group 1.Heart rate in group 3 was significantly slower than those in group 1 and group2 during the instant time after intubation.In group 3,blood pressure and RPP of instant time after intubation were significantly decreased than the base value,and the heart rate of instant time after intubation was greatly faster than the base value in group 2.Any complications related with intubation were not found after surgeries.Conclusion As the experience of learning gradually were gained,the intubating time by using shikani optical stylet through amnesia and analgesia induction method were shorter and shorter,the successful rates of the first attempt were gradually raised and irritations to the throat were notably alleviated.
