摘要
目的探讨老年慢性肺心病患者并发心律失常原因、分类及临床特点、合并症。方法回顾性分析我院呼吸内科确诊的320例老年肺心病患者,均进行心电图检查及其他实验室检查。结果 234例发生心律失常(占73.1%),其中两种或以上的心律失常72例(占30.8%),激动起源异常145例(占62.0%),传导异常89例(占38.0%),其中不完全性右束支阻滞多见(占73.2%);无心律失常组86例(占26.8%)。并发心律失常组存活198例(占85.0%),死亡36例(占15.0%);无心律失常组存活84例(占97.6%),死亡2例(占2.4%),两者存活率比较,差异具有显著性(P<0.01)。根据临床表现分为代偿期组133例(合并心律失常56例),失代偿期组187例(合并心律失常178例),两者发生率比较,差异具有显著性(P<0.01)。结论老年慢性肺心病患者并发心律失常发生率高,死亡率高,存活率低,预后差。失代偿期肺功能差者容易并发心律失常。及时、反复、多次行心电图检查,查清心律失常的原因,采取积极有效的综合治疗,是减少死亡率的重要措施。
Objective To discuss the reason and classified and clinical characteristic of old-aged chronic cor pulmonale patients complicated with arrhythmiao Methods Retrospectively analysis of 320 cases of old-aged chronic cor pulmonale patients of pneumology department which omplicated with arrhythmia was conducted.All were performed electrocardiographic examination and other labboratory examination. Results There were 234 cases with arrhythmia（73.1%）. Two types or obove 72 cases（30.8%）. Abnormal impulse formation 145 cases（62.0%）,Abnormalities of cardiac canduction were 86 cases（26.8%）. Chronic cor puhnonale patients concurrent arrythmia group survives 198 cases（85.0%）,36 cases were dead（15.0%）,the non-arrythmia group survived 84 cases（97.6%）,2 cases were dead （2.4%）,comparison with survival percentage,the difference has the significance （P〈0.01）. It was divided to compensated group （133 cases ,56 cases with arrhythmia） and decompensated group （187 cases, 178 cases with arrhythmia）according to clinical manifesta- tion. comparison with arrhythmia percentage,the difference has the significance （P〈0.01）. Conclusion Decompensated group of old-aged chronic eor pulmonale patients is easy to occur arrythmia. The old-aged concurrent arrythmia mortality rate is high,the surviva/ percentage is low, the prognosis is bad,prompt,repeatedly multiple electrocardiogram inspection, investigates thoroughly the arryth- mia the reason,It is important to adopt the complicacated effective treatment that can reduce the mortality rate .
