慢性肾功能衰竭血液灌流串联血液透析滤过清除瘦素、甲状旁腺素效果研究 被引量:9

摘要 目的探讨血液灌流串联血液透析滤过对慢性肾功能衰竭(chronic renal failure,CRF)患者体内瘦素、甲状旁腺素(parathyroid hormone,PTH)的清除效果。方法将40例CRF透析患者随机分为两组,A组20例,单纯行血液透析(hemodi-alysis,HD)治疗;B组20例,行血液灌流(hemoperfusion,HP)串联血液透析滤过(hemodiafiltration,HDF)治疗,分别于首次治疗、连续治疗4周前后测定两组血浆瘦素、PTH水平,对比并分析各指标间的相关性。结果 A组患者治疗前后瘦素、PTH水平差异均无统计学意义(P>0.05);B组首次治疗后与首次治疗前比较,末次治疗后与首次治疗后比较,差异均有统计学意义(P<0.05)。B组两种物质的清除率明显高于A组,差异有统计学意义(P<0.05)。结论血液灌流串联血液透析滤过能有效清除瘦素、PTH。 Objective To investigate the effect of hemoperfllsion and hemodiafihration on clearing the serum leptin and parathyroid hormone(PTH) in the patients. Methods Forty MH patients were divided randomly into groups A and B, 20 each: group A on hemodialysis(HD) and groups B on hemoperfusion and hemodiafiltration . Serum leptin and parathyroid hormone were detected in the two groups before and after treatment, and methods after 4 weeks. The results were obtained to analyze the correlation between the indexes. Results There were no significant differencs in leptin and parathyroid hormone in group A between before and after treatment ( P 〉 0.05 ) ; leptin and parathyroid hormone level decreased remarkably in groups B, compared between before and after the first time of treatment, and between the first time of treatment and the last time of treatment ( P 〈 0.05 ). The levels of cleanranee for serum leptin and serum parathyroid hormone in groups B were significantly higher than those in groups A (P 〈 0.05). Conclusion Hemoperfusion effectively clears leptin and PTH ,whiLe hemodialysis does not.
