摘要
目的：探讨正压无创机械通气治疗慢性阻塞性肺疾病急性加重呼吸衰竭期的治疗价值.方法：64例慢性阻塞性肺疾病急性加重呼吸衰竭随机平分为对照组和呼吸机组,对照组采用常规药物治疗,治疗组在此基础上加用正压无创机械通气治疗.结果：治疗组患者心率（HR）,呼吸频率（RR）均比治疗前减慢,pH 值和PaO2明显提高,PaCO2下降,各项指标与治疗前相比差异均有统计学意义（P〈0.05）;对照组除PaCO2有明显下降外,其他各项指标虽有所改善,但与治疗前比较无统计学意义（P〉0.05）.两组不良反应发生情况相比无显著性差异（P〉0.05）.结论：早期应用双水平气道正压机械通气辅助治疗慢性阻塞性肺疾病急性加重呼吸衰竭具有很好的疗效,能更快速缓解患者的临床症状,缩短住院日,是理想的治疗手段,值得临床推广使用.

Results：Treatment group patient heart rate （HR） ,breath frequency （RR） reduces speed compared to the treatment before,the pH value and the PaO2 distinct enhancement, the PaCO 2 drops,before each target and the treatment compares the difference to have statistics significance （P〈0.05） ;The control group has the obvious drop besides PaCO, although other each target has the improvement,but compares the non--statistics significance with the treatment before （P〉0. 05）. Two groups responded not good has the situation tCcompare the non--significance difference （P〉0.05）. Condusion .. The early time applies the double horizontal gas channel positive pressure machinery ventilation assistance treatment chronicity bloc- king lung disease acute aggravation breath failure to have the very good curative effect, can alleviate patient＇s clinical symptom fast, reduces in hospital date, is the ideal treatment method, is worth the clinical promoted use.
