摘要
目的:探讨连续性血液净化治疗糖尿病乳酸酸中毒的疗效。方法:对13例糖尿病乳酸性酸中毒的患者在常规吸氧、补液、控制血糖、抗感染、纠正水电解质酸碱紊乱、抗休克等的基础上,应用连续性静脉-静脉血液滤过治疗,观察分析患者治疗前后意识、血压、血乳酸、二氧化碳结合力、血pH值及病死率。结果:11例患者意识、血压、血乳酸、二氧化碳结合力、血pH值在96 h内恢复正常。治愈11例,死亡2例,死于多器官功能障碍。结论:连续性血液净化治疗糖尿病乳酸性酸中毒疗效确切,值得临床推广应用。
Objective： To evaluate the effect of continuous blood purification （CBP）on patients with diabetic lactic acidosis. Methods： 13 patients with lactic acidosis were treated with continuous veno-venous hemofiltration （CVVH） besides conventional treatments such as oxygen,fluid infusion,blood sugar control,anti-infection,correcting water electrolyte and acid-base imbalance and anti-shock. The consciousness, blood pressure, blood lactic acid, CO2CP and blood pH were observed before and after the treatment. Results： 11 of the 13 cases were cured,whose consciousness,blood pressure,blood lactic acid,CO2CP and blood pH return to normal in 96 hours,the other 2 cases died of MODS. Conclusion： CBP is effective in the treatment of diabetic lactic acidosis and worthy of clinic application.
