摘要
目的探讨高血压和血糖、血脂、肥胖等代谢性因素对代谢综合征(MS)患者左心室肥厚的影响。方法选择2006-2007年在我院就诊患者202例,采用血生化学方法测定所有病人空腹血糖、总胆固醇、三酰甘油、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)。按是否合并高血压及其他代谢因素分为:正常对照组(21例)、单纯高血压组(49例)、高血压合并血糖升高组(A组,19例)、高血压合并血脂紊乱组(B组,16例)、高血压合并肥胖组(C组,17例)、高血压合并血糖升高+血脂紊乱组(D组,18例)、高血压合并血糖升高+肥胖组(E组,17例)、高血压合并血脂紊乱+肥胖组(F组,21例)、高血压合并有3项代谢因素组(G组,24例)。采用超声心动图测量左心室舒张末期内径(LVEDd)、舒张末期左心室后壁厚度(LVPWT)、舒张末期室间隔厚度(IVST),计算左心室质量(LVM)和左心室质量指数(LVMI)。结果与正常对照组LVMI相比,单纯高血压组和高血压合并有一项和多项代谢因素组的LVMI均明显增高[(72.6±13.8)比(99.2±22.0)、(108.1±13.4)、(119.4±23.6)、(127.3±15.0)、(134.7±20.8)、(142.0±11.9)、(145.3±26.6)、(156.9±31.5)mm,均P<0.05];与单纯高血压组比较,高血压合并有一项和多项代谢因素组的LVMI均明显增高。高血压合并有两项和3项代谢因素组的LVMI高于高血压合并有一项代谢因素组。LVMI与空腹血糖、三酰甘油、总胆固醇、HDL-C、LDL-C和体质量指数均相关(r=0.33,r=0.19,r=0.17,r=-0.27,r=0.22,r=0.29;P<0.01~0.05)。多元线性逐步回归分析显示收缩压、空腹血糖、和HDL-C是影响左心室肥厚的独立因素(R2=0.311,P<0.01)。结论 MS患者随着高血压合并代谢因素的增多,左心室肥厚更为明显。收缩压、空腹血糖、HDL-C是左心室肥厚的独立影响因素。
Objective To investigate the effects of hypertension and metabolic factors such as plasma glucose,lipid,and obesity on the left ventricular hypertrophy in the patients with metabolic syndrome.Methods Fasting plasma glucose(FPG),serum total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C)and low density lipoprotein cholesterol(LDL-C)were detected in 202 patients who visited our hospital from 2006 to 2007.According to whether they suffered hypertension or other metabolic items,the subjects were divided into 9 groups:normal control group(n =21),essential hypertension(n =49),hypertension with hyperglycemia(group A,n =19),hypertension with dyslipidemia(group B,n =16),hypertension with obesity(group C,n =17),hypertension with hyperglycemia and dyslipidemia(group D,n =18),hypertension with hyperglycemia and obesity(group E,n =17),hypertension with dyslipidemia and obesity(group F,n =21),hypertension with hyperglycemia,obesity and dyslipidemia(group G,n =24).Left ventricular end-diastolic diameter(LVEDd),left ventricular posterior wall thickness(LVPWT)and interventricular septal thickness(IVST)were determined by echocardiogram.Left ventricular mass index(LVMI)was calculated based on these parameters.Results Compared with normal control group(72.6±13.8)mm,LVMI was significantly elevated(99.2±22.0)mm of essential hypertension,(108.1±13.4)mm of group A,(119.4±23.6)mm of group B,(127.3±15.0)mm of group C,(134.7±20.8)mm
